ROLE OVERVIEW and PURPOSE:
Neighborhood Healthcare is proud to partner with the Southwest Healthcare Medical Education Consortium to host Family Medicine residents during their continuity clinic rotations. Residents will train in a community health setting, gaining valuable outpatient experience while serving a diverse patient population.
**Please note, this opportunity is exclusively for residents enrolled in the Southwest Healthcare Medical Education Consortium Family Medicine Residency program. Applicants not currently in this program will not be considered.**
RESPONSIBILITIES:
As a Medical Resident at Neighborhood Healthcare, you will:
- Provide continuity outpatient care to an assigned patient panel using a Patient-Centered Medical Home model
- Deliver comprehensive primary care across all age groups, addressing preventive care, chronic disease management, and acute concerns
- Participate in innovative care delivery, including group visits, community-based outreach, and population health initiatives
- Collaborate with an interdisciplinary care team including physicians, nurses, behavioral health providers, and care coordinators
- Engage in quality improvement projects and apply evidence-based medicine to optimize patient outcomes
- Continue to develop your skills in leadership, patient advocacy, and culturally competent care
REQUIREMENTS:
- Current enrollment in the Southwest Healthcare Medical Education Consortium Family Medicine Residency program
- Commitment to scheduled continuity clinic days at Neighborhood Healthcare
- Strong interest in community-based, patient-centered primary care
