SUMMARY 

The Certified Nursing Assistant (CNA) performs varied and diverse work and is responsible for person-centered service to people with disabilities served by PDI. The CNA has knowledge of and involvement with areas and programs related to Medicaid. The CNA ensures the safety of people served and fellow employees in all operations. 


ESSENTIAL DUTIES AND RESPONSIBILITIES 

Person-Centered Support - CANDIDATE MUST BE CURRENTLY CERTIFIED

 

  • Develop relationships and understand the individuals served.
  • Support behavior management.
  • Follow safe practices and protocols, including medication administration, transfers, and personal care.
  • Adhere to infection control policies.
  • Provide physical support and assistance.
  • Document medical services and daily concerns/actions promptly.
  • Assist with activities of daily living (e.g., toileting, bathing, dressing, feeding).
  • Complete incident reports for reportable occurrences.
  • Engage with individuals to provide safe medical care.
  • Manage medical care and provide mentorship.

Respond to emergencies and liaise with emergency personnel.

  • Administer medications and manage G-tube use.
  • Maintain confidentiality per PDI policy and HIPAA.
  • Accurately administer and document medication per guidelines.
  • Perform and record blood glucose monitoring and report critical values.
  • Provide nutrition assistance, including G-tube feedings, and record intake.
  • Perform and document vital signs accurately.

 

Requirements

  

MINIMUM REQUIREMENTS FOR ALL PDI POSITIONS You must be age 18 or over to work at PDI. You must also pass the pre-employment screenings at any time and without notice while employed by PDI. You must be computer-literate, comfortable with technology with well-developed oral and written communication skills. You must be able to complete and pass First Aid, CPR, and other required training. 

If the position requires you to drive a PDI vehicle on company business, then you must possess a valid driver’s license and agree to a review of your motor vehicle record for addition to PDI’s authorized drivers list. In addition, if the position authorizes you to drive your vehicle on PDI business then you must provide proof of automobile insurance and proof of required automobile insurance limits. WILL ASSIST PROGRAM STAFF 


REQUIRED EDUCATION AND EXPERIENCE 

 High school diploma or general education degree (“GED”) required. 

The Certified Nurse Assistant (“CNA”) must be in good standing as credentialed by the Colorado State Board of Nursing


The CNA must have two (2) years experience working with people with disabilities in a day program or residential program setting or an equivalent combination of education and experience. The CNA will complete all required training including but not limited to cardiopulmonary resuscitation (“CPR”), First Aid, and required periodic refresher courses and continuing education units in the required periods. 


LANGUAGE ABILITY 

Ability to read an interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to clearly and legibly write routine reports and correspondence. Ability to speak effectively with people served and their support networks and co-workers. 


MATHEMATICS ABILITY 

Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. 


COMPUTER SKILLS 

Knowledge of word processing software, spreadsheet software and Internet software. Ability to learn and gain proficiency in other software and applications used by PDI. 


REASONING ABILITY 

Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several variables. 


WORK ENVIRONMENT 

The CNA works in the main PDI facility. Reasonable accommodations may be made to enable persons with disabilities to perform the essential functions. While performing the duties of this job, the CNA is occasionally exposed to outdoor weather conditions and occasionally exposed to wet or humid conditions (non-weather). The noise level in the work environment is usually moderate but can often be elevated and hectic. 


POSITION TYPE AND EXPECTED HOURS OF WORK 

This is a non-exempt, full-time position.  Hours will generally be 7:30 a.m. until 3:30 p.m. Monday through Friday. Hours may vary based on a variety of factors. An unpaid 30-minute meal period is required. 


PHYSICAL DEMANDS 

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable persons with disabilities to perform the essential functions. 


The CNA must regularly lift and /or move up to 50 pounds, frequently lift and/or move up to 100 pounds, and occasionally lift and/ or move more than 100 pounds with the aid of an assistive device. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this Job, the CNA is regularly required to reach with hands and arms and talk or hear. The CNA is frequently required to stand; walk; sit; use hands to finger, handle, or feel; climb or balance, and stoop, kneel, crouch, or crawl. The CNA is occasionally required to taste or smell food. 


TRAVEL 

The CNA has no out-of-town travel-related responsibilities but may have limited required local travel OTHER DUTIES

WILL ASSIST PROGRAM STAFF in travel.
Individual Entry• Traffic Control and Assistance to People Served

• Direct and closely observe vehicles in the parking lot and pick-up and drop-off areas

• Assist people served as needed in and out of vehicles and while moving in designated pedestrian areas

• Summon and assist people served in the afternoon as their rides arrive

• Communicate departure times to sign-out staff as needed


Lavatory Assistance 

• Assist people served with personal needs in the lavatory

• Launder clothes that are soiled during the day

• Launder bibs and other materials used during the day program


Lunchroom Management 

  • Supervise and support individuals during lunch breaks, including food preparation and setup.
  • Ensure lunchroom safety and monitor behaviors.
  • Assist individuals with personal needs, from full assistance to prompts.
  • Monitor and interact with individuals to enhance their quality of life.
  • Plan and assist in daily activities such as exercises and relaxation techniques.
  • Track accurate arrival and departure times for individuals.
  • Maintain accurate notes and monitor progress toward individual goals.
  • Plan activities to help maintain individuals' independence.
  • Assist with lunchroom management weekly.
Day Habilitation (Cognitive Skills, Fine Motor Skills, Gross Motor Skills)
  • Plan and implement activities to maintain and improve individuals' skills and social skills.
  • Engage individuals in daily activities.
  • Keep accurate notes on progress toward individualized goals.
  • Ensure the safety and well-being of individuals.
  • Assist with weekly lunchroom management.


Supported Living and Community Connections Program

  • Transport people served in company vehicles to off-site community resources as directed by the supervisor
  • Accompany and supervise people served at all times
  • Activities may include but are not limited to recreation, socialization, communication, skill-building, physical fitness and money management
  •  Days of week and hours of workday vary based on the person being served and overall program requirements
  • Follow PDI policies and procedures to be added to its authorized driver list before any activities related to the Supported Community Connections Program


Prevocational Services / Supported Community Employment
  • Plan and implement activities to improve skills and social skills.
  • Engage individuals in daily activities.
  • Track progress toward goals.
  • Ensure safety and well-being.
  • Assist with weekly lunchroom management.
