Critical Tasks:
- Perform nursing assessment and identify pertinent information to verbalize to the provider.
- Review and implement primary care provider orders
- Participate in ongoing education and training
- Educate patient regarding their healthcare and the healthcare system for patient/family to effectively care for patient
- Reduce the risk of infections
- Accept delegated tasks when necessary
Requirements
- Maintain proper hand-hygiene
- Use personal protective equipment (PPE) according to Center for Disease Control & Prevention (CDC) recommendations
- Perform low-level disinfection of medical equipment, devices and supplies
- Properly sterilize equipment and supplies
- Maintain supplies, including stocking and ordering new supplies
- Properly dispose of expired or damaged supplies, medical equipment and devices.
- Address patient telephone inquiries by evaluating and responding to the patient's problems and questions.
- Assist patient with scheduling appointments
- Follow up with patients who miss appointments.
- Confirm medical records are available in patient chart before follow-up appointments
- Obtain a patient's vital signs, pain level and weight.
- Screen patient for depression, changes in tobacco use, caffeine use, diet, health literacy level, preferred method of learning, and medication allergies.
- Provide written materials consistent with patient reason for visit as appropriate per evidence based practice (EBP)
- Provide (EBP) patient education videos, audio, and other selected material consistent with patient reason for visit or as requested by patient
- Provide copy of patient’s updated written medication list to patient healthcare team
- Encourage patients to update their immunization according to CDC recommendations by sending letter reminders
- Interview patient to determine medical problem/condition
- Collects samples, such as urine and sputum from patients for testing and follow-up needs with patients.
- Order diagnostic procedures and labs as directed by primary care provider with verbal order read back
- Dress wounds, apply hot or cold compresses as ordered.
- Observe patient(s) and report adverse reactions to medication or treatment to provider.
- Attend mandatory monthly meetings
- Locate and attend meetings, online webinars, conferences and training that emphasize your role/responsibilities
- Locate articles and stay up to date on latest evidence based practice regarding your role/responsibilities
- Complete yearly competencies as assigned by supervisor
- Ensure that a delegated task you receive is not complex, you understand the task, and you are adequately trained for the task before accepting the task
- Ensure that you have effectively completed a delegated task at least once before with confidence and that you can perform the activity correctly
- Ensure that you can perform a delegated task safely and patient is safe for you to carry out the task
- Ensure appropriate supervision by delegator is available during implementation of a delegated task
- Seek clarification and request additional training and guidance as needed for delegated tasks you receive
- Report care results to delegator in a timely manner upon reception of a delegated task
Metrics:
- Document patient assessment in patient’s health record prior to patient scheduled appointment or walk-in time slot with provider. Document with red-ink on patient superbill to identify changes or abnormal vital signs if provider or patient’s nurse is unavailable.
- Document nursing interventions, evaluations, and notification to patient provider to patient’s health record within a 24 hour time frame of patient’s visit.
- Document patient/family and healthcare team communication regarding appointments, labs, referrals, and medical records, including faxed/requested items, in patient’s health record within a 24 hour time frame of patient/family and healthcare team communication.
- Maintain record of certifications and trainings obtained within a calendar year
- Document assigned room cleaning and expired supply removal and return document to your supervisor on a monthly basis
- Schedule and hold monthly or quarterly rounding meeting with your direct supervisor
Experience, Training, and Qualifications needed to perform the job:
- Nurse assistant training program from an accredited college or university
- Certification within state of Utah
- Must be able to safely lift, push and pull fifty (50) pounds or more above waist
License/Certification:
- Must possess a valid driver’s license
- Must successfully pass a criminal background check
- Must obtain and maintain First Aid/Cardiopulmonary resuscitation (CPR) certification
- Must receive annual inoculation of flu vaccine
- Must receive annual tuberculin skin test
Experience, Training, and Qualifications needed to perform the job:
- Completion of a CNA program with Certification.
- One to two years’ experience as a CNA.
- Knowledge of Electronic Medical Records is a must.
In accordance with Navajo Nation and federal law, UNHS has implemented an Affirmative Actin Plan pursuant to the Navajo Preference in Employment Act. The NPEA has the following preferences:
1) Enrolled Navajo (must have a CIB).
2) Applicant married to an enrolled Navajo and who has lived on the NN for more than a year.
3) Everyone else (all other tribes and non-Indians).
As a Tribal Organization the Utah Navajo Health System, Inc. (UNHS) treats patients with high risk and underlying chronic medical conditions. Therefore, UNHS requires its employees to show proof of immunization prior to their employment with UNHS or during their employment. Below is a list of immunizations/vaccinations required to be employed or to continue employment with UNHS.
The immunizations/vaccinations include but not limited to following:
- MMR (Measles Rubeola, Mumps, Rubella): Documentation of two MMR vaccines (OR) Documentation of MMR titers.
- Complete Hepatitis B vaccine series (3 doses), documented proof of titers indicating immunity, or a declination that may be signed upon arrival.
- Proof of up-to-date PPD skin testing, if previously negative (OR) Proof of positive PPD skin test with the most recent chest x-ray and treatment history.
- Tetanus, diphtheria, and pertussis (Tdap).
- Varicella titers, history of varicella or varicella immunization series.
- Influenza vaccine for the current year.
- Any other vaccinations as requested.
