GENERAL SUMMARY:
The Community Monitor II provides individual GPS-based monitoring services to youth and families within a multi-agency, community-based collaborative service system. Services are delivered throughout Milwaukee County and select surrounding communities. Monitoring occurs across multiple environments including homes, schools, workplaces, and community settings.
This position offers 24/7 monitoring support for all youth assigned to the program, ensuring consistent supervision and accountability. In addition to monitoring duties, the Community Monitor II is responsible for delivering case management services to referred youth and families enrolled in the Level II Program. This includes coordinating services, supporting goal development, and collaborating with service providers to promote positive outcomes for youth and their families.
ESSENTIAL FUNCTIONS:
- Complete daily in-person community check-ins, including home and school visits, and document in appropriate system(s) per contract requirements.
- Review case files to maintain familiarity with youth history.
- Sustain timely responses to all inquiries youth, families, service providers and Milwaukee County Department of Children, Youth & Family Services (CYFS).
- Develop and maintain helping relationships with youth in program to encourage individual accountability. Complete all required documentation in an accurate and timely manner.
- Support the family decision-making process to empower the youth and family to avoid further law violations or continued involvement in the Juvenile Justice System.
- Provide GPS monitoring of youth according to program expectations and enter data promptly in Agency and GPS tracking databases.
- Maintain professional interactions with school staff to monitor daily school attendance, address school behavior, and the performance of each youth to improve the youth’s functioning and behavior.
- Ability to co-facilitate pro-social activities and supervise youth and family sessions.
- Presentation of reports to contracting entities including court officials.
- Prepare all documentation according to the program’s designated time frames.
- Travel within Milwaukee County to ensure youth’s full compliance with the court’s orders and curfews.
- Transport youth to court, as needed.
- Maintain all required trainings and certifications.
- Supports the goals of the agency mission/vision.
- Adheres to St. Charles Youth & Family Services Policies and Procedures and program hours.
- Other duties as assigned.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- High School diploma or equivalent; Associate’s degree in Human Services, Criminal Justice or related field preferred
- One year experience with programs that serve targeted population.
- Knowledge base of adolescent and family issues and dynamics.
- Familiarity with database systems with ability to enter data frequently and timely.
- Knowledge of Milwaukee County (Department of Human Services, Children’s Court, Milwaukee Public Schools, and Milwaukee community based youth serving agencies.)
- Knowledge of human growth and development theories.
- Ability to document all interactions and activities in a timely manner.
- Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
- Must possess a valid Wisconsin driver’s license and liability insurance and must meet agency’s driving requirements.
- Ability to use vehicles including own for work related duties according to agency’s insurance liability standards.
- Ability to accurately perform record keeping and documentation entries.
- Ability to perform tasks using MS Windows applications and agency database systems.
WORKING CONDITIONS
- Indoor/outdoor requirements.
- Evenings and weekend requirements.
- Travel 70% using company vehicles.
