What does this position look like~ The Residence LPN coordinate with Team Leader for ongoing appointment scheduling and medical service coordination for assigned residents. Completes all documentation in a timely manner. Supervises self-administration of medication. Provide skills training related to management of medical needs. Provide group and individual in home educational series targeted towards needs of individuals in residence program. Provides education to residence staff regarding client medical needs and scheduled appointment/procedure.

Demonstrates competency in delivering Motivational Interviewing (MI), effectively engaging individuals served through engagement, assessment, and intervention.

What you need to apply~

• Licensed Practical Nurse (LPN) license required, two years' experience with target population
• Excellent verbal and written communication skills.
• Ability to work a 40-hour work week.
• Possession of a valid New Jersey driver's license with a clean five-year driving record that meets organizational standards and qualifies for insurance coverage.
