Prairie View is seeking compassionate and resourceful individuals for the role of Patient Navigator. This position is ideal for candidates interested in care coordination and increasing accessibility to mental healthcare, especially those looking to gain hands-on experience in a supportive and growth-oriented environment. Key Responsibilities include assisting patients in navigating Prairie View’s array of comprehensive healthcare services. Patient Navigators also assess patients for barriers to treatment via Social Determinants of Health and are responsible for making and following up on referrals to community partners. This role is Prairie View’s primary source for assisting individuals with KanCare (Kansas Medicaid) applications.

Qualifications
- Minimum Education: Preferred experience working directly with behavioral health populations.
- Minimum Qualifications: Possess demonstrated interpersonal skills; ability to work with a wide variety of individuals with behavioral health needs served by a CCBHC and ability to react effectively in a wide variety of human service situations.
- Must have good listening skills, verbal skills, and written communication skills
- Must be able to observe patient demeanor and non-verbal communication for assessment purposes
- Must be at least 18 years of age
- Must be able and willing to work flexible hours and adjust work schedule to meet patient and organization needs
- Must hold and maintain State of Kansas Case Manager Certification and appropriate CEU’s where applicable
- Must be able to communicate effectively with referral sources, external care providers, and mental health center team members (i.e. case managers, therapists, physicians, community support workers, teachers, pastors, coaches)
- Completion of required trainings upon hire.
- Work independently, or as a productive member of a team to develop a therapeutic rapport with diverse individuals.
- Must be proficient at basic keyboarding/word-processing skills
- Must be able to pass Kansas Bureau of Investigation Screen, Central Registry Screen, and DCF (Kansas Dept. for Children & Families, formerly SRS) background check
- Basic proficiency in math
