Overview
CarePointe is AltaPointe's access-to-care department and the single point of entry into the majority of AltaPointe's programs and services. CarePointe is responsible for coordinating consumer care throughout our continuum, which may include facilitating hospital admissions and outpatient appointments, verifying insurance coverage, making discharge arrangements following hospitalization and providing referrals and linkage to other community resources. Through a brief telephone screening, trained staff members assess an individual’s needs for care, triage any crisis that the caller may be going through, and determine the most appropriate level of care and service. The team members' have behavioral healthcare experience and training. Their educational credentials include bachelor and graduate degrees in mental health disciplines or related fields.
Responsibilities
CarePointe Aftercare and Referral Specialist coordinate discharge planning and aftercare/intake appointments for consumers discharged from psychiatric hospitals, local hospitals and referred by community providers. They also review paperwork and populate financial, demographic and clinical EMR data.
Essential Functions
- Coordinates discharge planning and aftercare/intake appointments for consumers discharged from BayPointe, EastPointe and local and State hospitals.
- Reviews relevant paperwork and populates financial, demographic and clinical EMR data to ensure accuracy, completion and communication/dissemination of information.
- Provides follow up to AltaPointe programs and staff, local hospitals, consumers/families and community providers as needed to ensure consumer continuity of care.
- Provides referral information and appropriate linkage to community resources, ensuring appropriateness of level of care through use of clinical criteria as needed.
- Completes other duties as needed, including but limited to the compilation and reporting of UM/UR data, post-discharge wellness checks, state hospital compliance reporting, addressing mental health services email from AHS website, etc.
- Completes initial screenings for potential consumers according to risk factors/priority when needed, including potential hospital referrals
- Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
- Seeks supervision and consultation as needed
- Accepts and employs suggestions for improvement
- Actively works to enhance skills
- Documents in a timely fashion per AltaPointe policy
- Documents appropriate, complete, and comprehensive AltaPointe Access to Care assessments
- Documents in a clear and concise manner

Qualifications
Minimum Qualifications:
Bachelor’s degree in a mental health discipline; knowledge of hospital discharge protocols; knowledge of placement criteria; knowledge in specialties, outlined below, to be attained within six (6) months of employment:
- Knowledge of psychiatric disorders
- Basic knowledge of the nature of addiction
- Knowledge of crisis intervention
- Knowledge of community resources
- Knowledge of cultural diversity
- Knowledge of DMH contract compliance standards
