We consider it a privilege to make a meaningful difference, and we love what we do. As the Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:
- Within 3 weeks complete all training required to perform screens and eligibility assessments for the 1115 Waiver.
- Conduct screens and eligibility assessments of individuals for the 1115 Waiver program.
- Coordinate with the Program Director for the Caring Connections Program to ensure staff/volunteer coverage for the Caring Connections Program in each office. May provide coverage for the Caring Connections Program in the Cobleskill or Sidney offices as necessary.
- Complete all direct service tasks related to the Caring Connections and Rent Supplement Programs including the completion of intakes, initial assessments, referrals and education, advocacy, budgeting, screening, and providing material goods and financial assistance.
Case Manager Minimum Salary: $ 20.00/hour
Case Manager Maximum Salary: $ 20.00/hour
Case Manager Schedule: Monday - Wednesday, & Thursday: 8:30AM to 4:00PM; Friday 8:30AM to 6:30PM
Qualifications:
- High School Diploma or GED
- 1 - 3 years of experience working with a diverse population
- Valid driver’s license, proof of current car insurance and registration and access to a reliable, working vehicle
