Join our team at AMHC as a full-time Adult Behavioral Health Home Coordinator / Community Integration Specialist and be part of a mission-driven, multi-disciplinary team committed to whole-person wellness and recovery.
About the Role:
This vital role supports adults living with long-term psychiatric challenges by helping them develop life skills, access resources, and integrate fully into their communities. You’ll use a holistic, wellness-based approach guided by SAMHSA’s 8 Dimensions of Wellness: Emotional, Financial, Social, Spiritual, Occupational, Physical, Intellectual, and Environmental.
Working as part of a Behavioral Health Home team, you’ll provide support and coordination that bridges mental and physical health services—meeting people where they are, and helping them move toward where they want to be.
What You'll Do:
- Build strong, therapeutic relationships with clients, including those who may be difficult to engage.
- Support clients in setting wellness goals and building skills across key life areas.
- Provide case coordination and service planning aligned with individualized plans of care.
- Offer supportive counseling to help clients make decisions and maintain community stability.
- Lead or co-lead group sessions focusing on wellness, life skills, and psychoeducation.
- Support families and significant others, when appropriate, as part of the client’s care plan.
- Coordinate and accompany clients to community resources or medical appointments as needed.
- Collaborate with providers to ensure integrated care for both physical and behavioral health.
- Respond to crises when appropriate, providing support and stabilization.
- Meet or exceed established service delivery standards through in-person and telehealth contacts.
- Maintain accurate and timely documentation in accordance with agency policies and procedures.

Requirements
- An associate degree or equivalent credits in a Human Services field is required.
- Bachelor’s degree preferred.
- Must be certified at the full MHRT-C or provisional level.
- Use of a personal vehicle and an active driver’s license is required.
