CASE MANAGER – Hilo

Are you passionate about mental health care and ready for a career that changes lives? Join Mental Health Kokua (MHK) in beautiful Hilo, Hawaii, as a Behavioral Health Case Manager and help adults living with serious mental illness achieve stability, independence, and community connection. In this rewarding role, you’ll provide person-centered, trauma-informed care—offering support, guidance, and advocacy to ensure each client has the opportunity to begin again. Working alongside a dedicated team, you’ll coordinate services, promote recovery, and help prevent unnecessary hospitalizations.

This position is based in Hilo and involves work in both office and community settings, including client homes, hospitals, and public spaces.

As a nonprofit 501(c)(3) organization, MHK is PSLF-eligible for federal student loan forgiveness.

Case Manager Responsibilities

  • Comprehensive Assessments: Conduct behavioral health evaluations to identify strengths, needs, and service priorities.
  • Individualized Service Planning: Develop, implement, and update treatment plans tailored to each client’s recovery goals.
  • Care Coordination: Facilitate access to psychiatric, medical, housing, and social services; provide follow-up support after hospitalization to reduce re-admissions.
  • Documentation & Compliance: Maintain accurate clinical records, confirm insurance eligibility, and complete billing requirements promptly.
  • Collaboration: Participate in case reviews, supervision, and interdisciplinary team meetings to ensure quality, culturally responsive services.
  • Recovery Empowerment: Use strength-based, solution-focused strategies to help clients gain independence and resilience.
  • Adaptability & Quality Standards: Stay current with best practices while following MHK’s case management playbook and program guidelines.

Case Manager Requirements

  • Preferred: Bachelor’s degree in psychology, sociology, social work, human services, or related behavioral health field.
  • Preferred: 1.5+ years of experience in case management or human services; experience with co-occurring disorders a plus.
  • Required:
    • Valid Hawaii driver’s license, vehicle insurance, and current safety inspection
    • Current TB Clearance, CPR, and First Aid certifications
    • Ability to pass a criminal background check and drug screening
    • Ability to lift up to 25 lbs and travel throughout Hawaii Island for client visits and meetings
