Home Health Registered Nurse (Full-Time & Part-Time Opportunities)

Position Overview:
As a Home Health Registered Nurse, you will provide direct patient care and manage an assigned caseload of patients, working under the direction of the physician. You’ll play a key role in ensuring that patients receive the highest level of care in their own homes, from admission to developing individualized care plans.
Key Responsibilities:
- Collect patient medical histories and assess physical health, including vital signs, mental status, and more.
- Educate patients and families on nursing services and plan of care.
- Deliver skilled nursing care in accordance with the care plan, maintaining confidentiality in compliance with HIPAA regulations.
- Regularly evaluate the effectiveness of your nursing interventions.
- Submit timely and accurate patient notes/documentation.
- Develop and adjust nursing care plans, coordinating with the broader care team.
- Assess whether additional services (LPN, Home Health Aide, Personal Care Attendant) are necessary and arrange for their provision.
- Coordinate referrals for services such as physical therapy or medical social work, in collaboration with the Director of Clinical Services.
- Supervise and support LPNs, Home Health Aides, and Personal Care Attendants to ensure quality care.
Qualifications
Qualifications:
- Valid Registered Nurse (RN) license
- Experience with OASIS and WellSky (Kinser) preferred (training provided)
- Med-surge and wound care experience a plus
- Current TB test results (within the last 12 months) and CPR certification
- Physical (within the last 12 months)
- Valid driver’s license
- Ability to attend orientation at our Taunton office
