SUMMARY
Provides services to adult mental health, child, and adolescent clients. Assists in developing, evaluating, and coordinating treatment plans and recovery services.
DUTIES
Establish a collaborative, respectful, and outcome focused relationship with the individual receiving services.
Collect and review intake and assessment documents, and other pertinent materials.
Develop, with the individual, and the input of the recovery team, the individual service plan (ISP).
Coordinate and implement the ISP in accordance with the following State rules: 1) assist the individual in the development of a recovery plan 2) develop a functional assessment of individual’s strengths and needs toward achieving recovery goals 3) assist the individual in identifying recovery goals and objectives 4) identify services to be provided to achieve the goals and objectives and either provide those services or arrange for their provision as appropriate within the scope of behavioral health services; and, 5) assist the individual in identifying natural supports, family or non-behavioral health resources toward achieving the recovery goals.
Assist in the maintenance and coordination of the clinical record. Has responsibility for: 1) ensuring that the clinical record is complete and up to date; 2) documentation of progress towards the goals and objectives
of the ISP; (3) documentation of services and case management provided; and 4) documentation that services identified on the ISP have been provided, or if not, documentation of the reasons why services were not provided.
Provide services through face-to-face contacts, telephone contacts, and collateral contacts providing community and clinic-based supports.
Monitor with the recovery team the individual's receipt of behavioral health services and response to those services and collaborate with the individual to determine the need for changes in or termination of the ISP. Plan for and develop changes in the ISP as needed and identified.
Establish, as appropriate and with appropriate consents, a relationship with family members and care providers of the individual receiving services.
As appropriate, and with consent, contact family members, providers and other agencies for information related to the individual behavioral health needs.
Interact with all components of the behavioral health delivery system, the primary care physician, and other members of the recovery team.
Obtain appropriate prior authorization of inpatient, partial hospitalization, residential and other services that require prior authorization.
Assist in identifying third party liability.
Act as the hospital liaison which includes assisting emergency room diversion, plan / options.
Performs other duties as assigned.

REQUIREMENTS
A master's degree or bachelor's degree in a field related to behavioral health,
Is a registered nurse.
Is a physician assistant who is not working as a medical practitioner
Has a bachelor's degree and at least one year of full-time behavioral health work experience,
Has an associate's degree and at least two years of full-time behavioral health work experience,
Has a high school diploma or high school equivalency diploma and:18 credit hours of post-high school education in a field related to behavioral health completed no more than four years before
the date the individual begins providing behavioral health services and two years of full-time behavioral health work experience.
Four years of full-time behavioral health work experience
Is licensed as a practical nurse, according to A.R.S. Title 32, Chapter 15, with at least two years of full-time behavioral health work experience.
Knowledge, Skills, Abilities
Demonstrated competency in Managed Care systems and provider networks.
Demonstrated competency in the adult recovery model.
Demonstrated competency in the philosophy of behavioral health programs.
Demonstrated competency in case management roles and responsibilities.
Demonstrated competency in case management procedures and allocation of resources.
Demonstrated competency in the characteristics of behavioral health issues and levels of severity.
Other Potential Qualifications
Reliable transportation, Valid AZ driver’s license, automobile insurance, and good driving record.
Proof of physical exam, TB skin Test, fingerprint clearance (if working with children)
