Description

Description: The Outreach RN / Outreach Coordinator is to maintain the mobile units and the schedules for the mobile units. The Outreach Nurse will also contact patients with test results (STI, labs, etc.), assist with HCV testing and treatment protocols and coordinate treatment and care with the clinician. In this role, the RN exhibits a professional attitude in dealing with our patients and staff at all times.

General Duties and Responsibilities

Outreach Coordinator:

  • Ensure the van gets to locations per the outreach schedule abiding by safe driving laws

  • Maintain the schedule for outreach events

  • Ensure enough and proper staff are at each event

  • Schedule events with local businesses throughout the city of Pittsburgh and surrounding areas

    RN Duties:

  • Utilize the nursing process to assess, plan and implement, and evaluate patient care

  • Performs interviews to identify specific patient needs

  • Collects, analyzes, and interprets information from healthcare members and documents as necessary

  • Collect patient health history

  • Counsel and educate patients and their families on treatment plans pertaining to HCV, HIV, STI’s, MAT and Narcan training

  • Notify patients of positive and abnormal results and coordinate treatment and care ASAP

  • Support clinician with any assigned duties, i.e., prescription refills etc.

  • Understand and educate patients on HARM reduction techniques

  • Administer medication, change wound dressings and performs other treatment options

  • Read and interpret patient lab results, making treatment decision where appropriate

  • Consult with clinicians to decide on the best treatment plan for patients

  • Research ways to improve healthcare processes and patient outcomes through data obtained/analytical tasks/and charting tasks related to patient information

  • Communicate with patients about their status and condition through emails, phone and face-to-face conversations

  • Performing administrative tasks, such as ordering supplies

  • Ensures all specimens have a corresponding State Sheet

  • Ensures all specimens are labled, spun, piped and placed in the refrigerator at NS for processing and shipment

  • Draw blood and label appropriately per office protocols

  • Oversee routing labs/immunizations/testing

  • Chart short description of each visit for Provider or MA visit.

  • Perform phlebotomy

  • Obtain vitals including height, weight, BP, pain level, pulse, and respiratory rate.

  • Obtain labs as ordered – properly label samples with the first initial, last name and birthdate as confirmed with patient.

  • Collect samples including urine, throat swab and rectal swab as ordered.

  • Properly log patient information into EKG and perform EKG successfully.

  • Any other task needed to be done by the Medical Assistant.

    Medical Case Manager Duties:

  • Help patients apply for insurance and medical assistance for our patient population

    Maintain the Central Outreach Mobile units

  • Schedule maintenance

  • Clean and organize the inside.

  • Properly stock the mobile unit with supplies

  • Wash the outside as necessary.

    Maintain the schedule of the mobile units for outreach events

  • Ensure the mobile unit is staffed and where it needs to be for outreach events.

  • Schedule outreach events with bars, events, etc.

  • Develop a process for requesting the mobile unit for outreach events

  • Track patients tested and volunteers for outreach events

    Additional Items

  • Need to make sure each patient is comfortable with their care.

  • Need to be culturally competent while working, including being gender sensitive and pronoun aware.

  • Develop relationships to ensure patients are comfortable with their medical care.

  • Always maintain HIPAA compliance.

  • Be able to work with a team of medical assistants, clinicians, and administrative staff.

  • Make sure to understand the employee handbook and follow all rules and guidelines outlined in the handbook.

  • Always maintain a professional and non-judgmental attitude.

Qualifications


Minimum Qualifications:

 

  • Active Ohio Registered Nursing License

  • CPR certification

 

 

 

Requirements

  • Clean OH Driver’s License

  • Ability to drive larger vehicles

  • Technical skills with van equipment

  • Emphasis on HARM reduction

  • Knowledge of basic computer skills, Excel, Word and how to use Electronic Medical Records

  • Experience with patient centered care

  • Punctuality and impeccable attendance

  • Strong communication skills with both professionals and patients from all demographics

  • Non-judgmental approach to patient care

  • Technical skills with required office equipment

  • Patience and great attention to detail

  • Completely understand the healthcare privacy laws as outlined in HIPAA
