Position Overview
This position assesses the functioning of the individual Independent Living foster care youth and his or her support team; sets treatment goals and plans interventions with the treatment team, youth, and support team in pursuit of permanency; coordinates, implements, and delivers the agreed-upon services; determines the youth’s progress to evaluate the effectiveness of the interventions and plans. This role also provides support, assistance, and advocates and coordinates services in the community.
Responsibilities:
- Ensures all activities adhere to agency values, with unwavering commitment to trauma-responsive care.
• Uphold unconditional positive regard for youth in care, separating the youth from their behavior, in all
interactions/conversations/repots with and about youth.
• Provides case management services for all assigned clients and families as required by MDHHS policy
• Develops treatment goals, primary objectives and action plans based upon the client’s strengths, needs,
abilities and preferences, and evaluates effectiveness of treatment planning, interventions, and improvement
of family functioning
• Provides supportive services that fosters life skill development and independence for clients in independent
living programs
• Coordinates activities between professionals involved with ILP youth, and provides input and direction for
treatment planning
• Complete required face to face contacts, medical and dentals, and service plans per Key Performance
Indicators set by the MDHHS guidelines
• Required to transport clients as needed
• Provides outreach assistance and coordination to families and clients to ensure needs are met; coordinates
visits to schools, courts, referring agency and other agencies to facilitate interventions and treatment
• Writes comprehensive individualized service plans, court reports, and other documents as required per policy
• Will maintain compliance with performance standards, DHHS policy and contractual responsibility
• Will attend staff meetings and all other mandatory trainings as required by HCS
• Actively participates in weekly supervision with the positions immediate supervisor.
• May be required to work shifts (only applicable for those covering the independent living plus program)
• Responsible for teaching, modeling, practicing, supporting, and articulating the Agency’s treatment
philosophy
• Performs other duties related to the essential duties of the position and within the individual’s capabilities as
assigned by one’s
• Provide 24-hour crisis on call availability to treatment team according to on call schedule
• May be required to work shifts on an as needed basis

- Bachelor’s degree in one of the following:
- Interdisciplinary Studies in Social Sciences; Education; Community Development; Law Enforcement; Forensic Psychology; Gerontology; Special Education; Education of the emotionally disturbed; Education of the gifted; Behavioral Science; Community Services; Counseling (Psychology); Criminal Justice (Administration); Early Childhood Studies; Family Ecology; Family Life Education (Spring Arbor University); Family Studies; Family land/or Child Development; Guidance/School Counseling; Human Development and Family Studies (Michigan State University); Human Services; Psychology; Social Work; Sociology
- (OR)
- Bachelor’s degree of another major with at least 30 semester credits in one or a combination of the approved degrees. Experience working in child welfare is preferred.
