Hospice of the East Bay (HEB) is a community-based hospice and palliative care program providing services in the home and at our six-bed Hospice facility for terminally ill patients and those in need of palliative care.  Care is given to patients and families by an interdisciplinary group.  Bereavement care is provided to the family for a minimum of one year after the death.  Hospice and palliative care offer a sponsored care program to provide services regardless of patient’s ability to pay. Hospice East Bay has been certified the past five years in a row as a "Great Place to Work". 

Coverage Areas: American Canyon, Hercules, Richmond, Oakland, Castro Valley, Walnut Creek, Pleasanton, Antioch, Brentwood and the surrounding cities in the area.

POSITION SUMMARY:  

Reporting to the Clinical Services Manager, this position is the acting initial point of contact for patients and families entering hospice services. This position involves assessing patients' eligibility for hospice care, explaining services and benefits, completing admission paperwork, and ensuring a smooth transition into hospice care. The Hospice Admission Nurse works closely with the interdisciplinary teams to provide comprehensive, compassionate care tailored to the needs of patients with life-limiting illnesses. Primarily works in patients’ homes, hospice facilities, or healthcare settings. Requires travel within the service area. This position offers a rewarding opportunity to make a meaningful difference in the lives of patients and families during a critical time, providing compassionate care and support.

ESSENTIAL FUNCTIONS:

  • Conduct comprehensive assessments of patients referred to hospice care, evaluating physical, emotional, social, and spiritual needs.
  • Determine eligibility for hospice services based on clinical criteria and LCD guidelines.      
  • Explain hospice philosophy, services, and benefits to patients and families.
  • Provide education on disease progression, symptom management, and end-of-life care options.
  • Complete all required admission paperwork accurately and efficiently.
  • Obtain necessary consents and authorizations.
  • Coordinate with physicians, referral sources, and other healthcare providers to facilitate the admission process.
  • Ensure the care plan addresses the patient's and family’s goals, preferences, and needs.
  • Maintain accurate, timely, and thorough documentation of patient assessments, care plans, and services provided.
  • Ensure compliance with regulatory and organizational standards.
  • Provide emotional support and advocacy for patients and families during the admission    process.
  • Address any concerns or questions they may have.
  • Stay current with hospice best practices, clinical guidelines, and regulatory requirements.
  • Participate in ongoing education and training to enhance skills and knowledge.

 POSITION REQUIREMENTS: 

  • Demonstrates the skills, knowledge and abilities necessary to be successful in the job as outlined in the position requirements
  • Excellent interpersonal and communication skills.
  • Strong clinical assessment and critical thinking skills.
  • Knowledge and understanding of the hospice philosophy and hospice criteria for admission and the admission process.
  • Knowledge of private insurance, Medicare and Medi-Cal regulations
  • Knowledge of State and Federal regulations and CHAP standards
  • Compassionate and empathetic approach to patient and family care.
  • Ability to work independently and as part of an interdisciplinary team.
  • Proficiency in electronic medical records (EMR) and documentation.
  • Ability to lift and transfer patients.
  • Prolonged periods of walking, standing, and sitting.
  • Ability to perform physical tasks such as bending, stooping, and reaching.  


EXPERIENCE AND EDUCATION:
 

  • Valid California registered nursing license
  • California driver’s license and insurance
  • Minimum of 2 years of nursing experience, preferably in hospice, palliative care, or home health.
  • Experience in conducting patient assessments and care planning.
  • Certified Hospice and Palliative Nurse (CHPN) preferred, but not required.
