Position Description:

The Mental Health Rehabilitation Worker will provide crisis assessment and stabilization services to recipients in the Willow Tree program. Support recipient with learning, practicing, and implementing these skills to stabilize mental health symptoms and restore level of functioning. Clinical Supervision provided by Willow Tree Clinical Director - Mental Health Professional (MHP) and/or Mental Health Practitioner (MHPR).


Primary Job Responsibilities:

  •  Communicates personal and cultural respect to recipients, family members, and co-workers
  •  Completes intake form, privacy notice, releases, and other required documentation with recipient upon entering the program
  •  Assists recipients in the orientation of programming guidelines and house expectations
  •  Attends weekly clinical staff meetings with Stellher Coordinator and Clinical Supervisor and completes all training assignments for continuing education
  •  Teaches and coaches recipients on the skills of crisis management and independent living
  •  Performs work collaboratively with recipients, family members, Stellher MHP, case managers, and other support staff
  •  Follows Daily Planner as directed by MHPR; implements assigned interventions in recipient’s ITP
  •  Conducts individual, family and group skills training to adults with SPMI
  •  Maintains detailed logging on services provided and the recipient’s response to interventions, incident reports, concise and thorough progress notes, and other required documentation
  •  Accommodates recipient in scheduling mental health service appointments and related activities to support treatment progress
  •  Assists each recipient in the administration of medications as prescribed by a licensed physician
  •  Possesses knowledge of and ability to access community services for each recipient
  •  Assists recipient in utilizing an established crisis management plan when needed
  •  Implements approved de-escalation techniques as directed by the Stellher MHP or MHPR
  •  Carries out responsibilities of the Crisis Phone Responder to include gathering information on the Phone Responder Intake Form during the initial crisis call, coordinating services with the Mobile Crisis MHPR on call, and entering information into the Community Crisis Response database
  •  Follows Stellher Human Services, Inc. policies and procedures regarding privacy, incident reporting, recipient grievances, and vulnerable adult reporting
  •  Maintains a healthy, clean, organized environment for recipients to receive services
  •  Establishes and maintains good working relationships with clients and coworkers
  •  Treats clients and coworkers with dignity and respect
  •  Works as part of a team to promote a culture of continuous improvement and commitment to the mission of the organization
  •  Carries out other duties as assigned


Requirements

Qualifications:

  •  At least 21 years of age, possess a high school diploma or equivalent, and meet the Minnesota Department of Human Services health care rules for qualifications of a mental health rehabilitation worker. (www.dhs.state.mn.us) AND  Have successfully completed 30 hours of training during the two years prior to hiring in each of the following areas (recipient rights, recipient-centered individual treatment planning, behavioral terminology, mental illness, co-occurring mental illness and substance abuse, psychotropic medications and side effects, functional assessment, local community resources, adult vulnerability, recipient confidentiality)

 As a Mental Health Rehabilitation Worker, you must also meet one of the following:

  • Has an associate of arts degree in one of the behavioral sciences, OR
  • Is a registered nurse without a bachelor’s degree, OR
  • Within the previous 10 years has had one of the following:
    • Three years personal life experience with serious and persistent mental illness, OR
    • Three years personal life experience as primary caregiver to an adult with a serious mental illness or brain injury, OR
    • 4,000 hours supervised paid work experience in the delivery of mental health services to adults with a serious mental illness or brain injury
  •  Demonstrated ability to interact with individuals from diverse cultural and socioeconomic backgrounds.
  •  Able to work independently within established guidelines and with limited supervision or assistance while communicating clearly and effectively with other professionals in verbal and written formats.
  •  Personal or professional experience working with individuals who have severe and persistent mental illness (SPMI) as well as knowledge of and ability to intervene appropriately in crisis prevention and de-escalation when necessary.
  •  Flexible in meeting scheduling needs, preserves confidentiality of recipient information, and institutes appropriate boundaries.

Physical Requirements:

  •  Prolonged periods of sitting at a desk and working on a computer.
  •  Must be able to lift 15 pounds at times.
  •  Must be able to access and navigate each department at the organization’s facilities.
Salary Description
$17.00 - $19.00, plus shift differential pay
