JOB SUMMARY

This position will provide Prevention navigation, including but not limited to HIV/STI testing, pre-/post-test counseling, condom education and distribution, referrals, and/or linkage to prevention and essential services for high-risk negative persons and persons living with HIV. This position will provide rapid entry to HIV care services at PIHC, off-site, and through self-based testing in a confidential, one-on-one session with clients while maintaining the pre-counseling protocol of no more than 30 minutes on average for the session. This position will support the PIHC PrEP program fully by providing PrEP and PEP education; linkage to same-day services; retention and reengagement to PrEP care; and administrative support. Staff will utilize established Rapid Entry and Re-engagement in Care protocols for linkage to care and prevention and essential services. This position will track linkage or re-engagement for up to 90 days to ensure clients access first medical appointment; document linkage and navigation services, notify local health department of all HIV positive clients for Partner Services. This position reports directly to the Prevention PrEP Program Manager and relates to targeted Prevention program objectives.

Requirements

ESSENTIAL FUNCTIONS:


Duties and Responsibilities


• Conducts HIV Counseling (CTL) sessions in a non-judgmental manner and with fidelity to the core elements and core components of the HIV Prevention Counseling standards.

• Conducts off-site testing in a sex positive manner in non-traditional settings during non-traditional work hours.

• Provide effective condom education. Distribute condoms to target HIV-infected persons and persons at highest risk of acquiring HIV.

• Provides education on pre-exposure prophylaxis (PrEP) and ensures persons eligible for PrEP, if interested, are linked to PrEP.

• Be able to answer all questions about PrEP for both agency staff and clients, attend community events to promote PrEP, and assist with PrEP marketing strategies.

• Provide PrEP navigation and linkage, including screening for eligibility, completing all required documentation and paperwork, and collecting all required fees.

• Coordinate enrollment in PIHC’s Rapid Entry Program for the following patients: newly diagnosed with HIV, living with HIV but never in care, or not currently engaged in care.

• Assess potential barriers to PrEP care and the impact that stigma has on accessing PrEP.

• Provides education about the importance of medication adherence for persons receiving PrEP medical care.

• Provides referrals to medication engagement services (ME) about the importance of HIV medical care and medication adherence to persons living with HIV.

• Collaborate/consult with PIHC clinic staff, medical case managers, and Behavioral Health staff for care coordination

• Schedule medical appointments (labs, clinic visits) for patients with the providers and/or nurses, utilizing the Electronic Medical Record (EMR) system.

• Enter clients into the electronic medical record (EMR).

• Conduct routine follow-up calls with PrEP patients and patients receiving HIV medical care.

• Attend required prevention navigation training regularly, as well as other applicable agency trainings.

• Abide by all state, health district, and agency policies regarding confidentiality of patient information.

• Follows all agency guidelines relating to HIV testing, PrEP protocols, rapid entry services, incentive distribution, and documentation of all program data.

• Perform other duties as assigned



Knowledge, Skills, and Abilities


Must be fluent in both written and spoken Spanish and English, with strong reading comprehension in both languages.

• Possess excellent communication skills (writing, spelling, listening, and speaking)

• Possess excellent organizational skills

• Ability to work independently as well as part of a team.

• Ability to resolve problems and make decisions.

• Ability to respond to assigned tasks in a timely manner

• Utilizes good management techniques and time management.

• Ability to follow through with completion of assigned tasks.

• Proficiency in Microsoft Suites.

• Abides by all policies and procedures concerning personnel, including conflict of interest policy as outlined in the Employee Handbook. 



Minimum Qualifications


• MINIMUM Bachelor’s degree from an accredited institution, in a public health, counseling, psychology, sociology or related field



Experience


• Two years of experience working in the field of HIV, PrEP or public health; or

• Any equivalent combination of proven training and experience that provides the required knowledge, skills, and abilities to perform this job.
