🎯 What You’ll Do in This Role:
We are seeking a compassionate and skilled nurse to join our PACE team! This role is responsible for providing both direct and indirect nursing care to PACE participants. While working under the direct and indirect supervision of the Clinic Supervisor or designee, this nurse operates independently, applying advanced nursing knowledge and clinical judgment. The ideal candidate is confident managing complex clinical situations and committed to delivering high-quality, person-centered care to older adults.
Primary Responsibilities:
Assessment
- Collect patient data and complete required forms per PACE standards.
- Serve as backup for Triage Nurse.
- Perform fall assessments in collaboration with Physical Therapy (PT).
- Identify participant problems/needs and set priorities.
- Recognize issues requiring referral or follow-up.
- Interpret and record diagnostic results; conduct advanced nursing assessments using critical thinking.
- Participate as a member of the Interdisciplinary Team and in participant care planning meetings.
- Conduct in-home assessments of prospective participants as assigned.
Leadership
- Adhere to established clinic workflows and operations under direction of Nurse Manager/Medical Director.
- Attend clinic meetings and huddles as assigned.
- Delegate nursing care using nursing process and scope of practice.
- Integrate cost-effective measures into nursing practice.
- Recognize and document unit issues and take appropriate action.
- Actively participate in Quality Improvement (QI) processes.
- Comply with safety policies and report unsafe working conditions.
- Address and resolve ineffective patient care management; complete necessary documentation.
- Fulfill on-call responsibilities as assigned.
Planning Care
- Complete initial, semi-annual, and annual assessments of participants prior to team meetings.
- Develop comprehensive care plans that include interdisciplinary input and continuing care needs.
- Involve participant and/or family in developing or revising care plans.
- Make referrals to appropriate interdisciplinary support services.
- Communicate participant status and care changes to team members.
Implementing & Managing Care
- Ensure care delivery aligns with established standards and protocols.
- Administer treatments/procedures (e.g., IV therapy, tube feeding, wound care) as prescribed.
- Manage participant medications, including reconciliation, communication with pharmacy, and education.
- Provide participant and family education for 24-hour care coordination.
- Recognize emergency situations and initiate appropriate interventions.
- Coordinate care with other healthcare personnel and evaluate outcomes.
- Respect participant rights and manage ethical issues appropriately.
- Maintain medication room, stock supplies, and dispose of expired items per policy.
- Track hospitalizations, admissions, and discharges; communicate with hospitals and document accordingly.
- Maintain timely, complete, and accurate documentation of participant care.
- Report and record infections per protocol; notify Nurse Manager and QA Director.
- Attend Nursing Facility Care Conferences as assigned.
- Conduct home visits for participant assessment and care interventions.
✅ What You’ll Need:
Required:
- Graduate of an accredited School of Nursing.
- Licensed Registered Nurse in the State of Michigan.
- CPR certified or willing to obtain upon hire.
Desired Qualifications:
- Three (3) years of experience as a practicing nurse in a health-related field.
- At least one (1) year of experience working with the frail or elderly population.
- Must have reliable transportation and possess a valid and current driver's license.
- Position requires the ability to lift and move objects weighting up to 50 pounds without assistance.
