- Assists patients and families to adjust to illness and lifestyle changesrelated to diagnosis, illness and treatment.
- Participates in screening for Distress through appropriate tools and provides timely interventions to address psychosocial distress.
- Completes psychosocial assessment and works with the patient and family members to develop a plan for intervention based on mutually agreed upon goals to maximize functioning through treatment and recovery.
- Provide for or arrange for individual, family and group counseling and other appropriate interventions to alleviate distress and enhance functioning.
- Identifies and utilizes resources to assist patient to adjust to changes in function, maximize independence and functioning.
- Assists in patient care planning to ensure patient has resources to meet the psychosocial, emotional and practical needs of a cancer illness.
- Evaluates the ongoing medical and psychosocial needs for home care
- planning in order to set up appropriate community services.
- Evaluates the current treatment plan, available social service programs and formulates a pertinent psychosocial care plan that will provide maximum benefit for each patient.
- Assists patient/family in the procurement of essential equipment to allow
- optimum and safe function at home.
- Assists patient/family in the decision making process leading to the
- selection of and placement in an appropriate long term care facility if appropriate.
- Facilitates discussion of end of life issues and provides resources and education to patient and family to allow for informed decision making and reduced distress
- Provides information and referral for continuing care and crisis
- intervention.
- Provides education to the patient, family and team regarding specialized cancer related community resources and programming related to the individual needs of the patient.
- Refers voluntary community based services to management such as community food pantries, charitable financial support organizations, as appropriate to enhance coping and provide for immediate patient and family needs.
- Functions as advocate for patients as they interact with appropriate community agencies.
- Evaluates the appropriateness of community services as possible and develop integrated plans to meet patient needs.
- Works cooperatively with co-workers and communicates effectively with
- departmental staff; other departmental personnel and health care
- facilities; physicians, patients and visitors.
- Acts in accordance with departmental policy and procedure.
- Functions within the professional National Association of Social Workers (NASW) Code of Ethics. Follows the Associate of Oncology Social Work Standards of Practice.
- Is available to all clinical areas in the Oncology Department for Social Work services as needed and assigned.
- Documents all OSW interactions with team, patient and family in the Electronic Medical Record for appropriate team communication, collaboration and continuity of care.
- Maintains monthly statistical data.
- Observes all safety, infection and environmental control procedures.
- Functions in such a way as to meet departmental quality assurance and process improvement programming.
- Attends and actively participates in meetings and serves on committees as
- requested.
- As assigned, participates in Tumor Conferences, Cancer Committee meetings and other Oncology related on-site meetings.
- Performs other work related duties as requested/directed.

- Master's degree in Social Work (MSW) and State licensure (L.I.C.S.W.)
- Minimum of two years Clinical Social Work in a health care setting
- Experience with Oncology patients preferred
- OSW0C Credential preferred
