Pay Scale: Pay is based off education, experience, and certification level. Levels are; Peer Support/Para Professional: $16-$17/hr.
AP:$17-$20/hr.
QP:$21.63-$23.07/hr. 

 

Available Shifts: 11am-8pm, Monday-Friday

 

Summary: This is a split position, requiring excellent ability to multi-task and prioritize tasks. Responsible for providing support and care to meet the physical and psychological needs of consumers through providing direct and indirect care. BHUC is a 24-hour behavioral health urgent care service. Also responsible for providing SOAR-related services within the BHUC. This includes assisting with submitting a complete and thorough SSI/SSDI application packet to SSA, assisting all Alliance state-funded members being seen at the BHUC with the application process, either virtually or in person. All completed applications would then be followed through final decision. This position can only be used to assist Alliance Health state-funded members through the SI/SSDI application process. Case management services can be provided only to Alliance Tailored Plan or state-funded members.

 

Essential Duties and Responsibilities:

  • Provides supervision and monitoring of each consumer. Assist with and model appropriate daily living skills.
  • Document any reported or observed symptoms of consumers and report to the team.
  • Participate in Shift Report
  • Provide transportation
  • Work with referral sources and community partners to identify applicants through team meetings,
    outreach, and referrals
  • Complete interviews with individuals to gather information to complete SSI/SSDI applications
  • Gather medical records and other information to complete SSI/SSDI applications
  • Write SOAR Medical Summary Reports for individual applications
  • Accompany individuals to appointments at the Social Security Administration
  • Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the
    application
  • Coordinate case management services with partners and help with providing case management services
    to individuals when needed
  • Assist the team with administrative tasks as needed
  • All other duties as assigned by supervisor

 

Special Attributes:  Strong interpersonal skills and belief in consumer’s ability to recover. Strong writing skills, as this position requires written medical reports and assistance with other reporting. Minimal supervision regarding use of time, able to prioritize work assignments. Ability to communicate effectively with professionals and clients/families. Ability to make sound decisions in emergency situations. A desire to work with individuals with severe mental illness and substance abuse issues.

 

Education and/or Experience:

Tech II:

  • QP- requires bachelor's degree in social work, Counseling or another Human Services field. Qualified Professional (QP) required.
  • A background in DSS, or other agencies that assist with Medicaid, SSI/SSDI is preferred for this position.
