Optim Medical Center Tattnall is seeking a PRN Rehab Patient Advocate for the Reidsville, GA location. 

JOB TITLE: Patient Advocate

TYPICAL WORKING CONDITIONS: Office environment with occasional evening or weekend hours. This position requires extensive sitting at a work station while performing office task at the computer. This individual should be competent in use of computer systems including word and Excel. There will be some stress related to dealing with concerns of patients and/or staff members.

GENERAL SUMMARY OF DUTIES: The Patient Advocate has access to the incoming scripts that the Physician/Medical Assistant drops into the message center. This person will call the qualifying patients and arrange set up of the initial evaluations. This person is also collecting data from referrals for future review.

EXAMPLES OF DUTIES: (This list may not include all of the duties assigned, and may vary from location.)

· Print Physical Therapy Scripts from SRS mail system daily

· Track daily scripts per Optim Physician

· Collect various data from patient referrals

· Call Surgery Center Post-Op referrals to schedule for Therapy

· Scan documents into patient charts

· Ensures new patient information is completed and scanned into patients chart

· Reschedules therapy appointments as needed

· Answers therapy department phone calls to schedule, reschedule, answer questions, or reroute calls pertaining to therapy. Checks messages daily and regularly. Return calls within daily, but no later than 1-2 business days. Keeps an accurate phone log.

ADDITIONAL RESPONSIBLITIES: (As necessary)

· Checks in/out incoming PT/ OT patients

· Prepares all new patient paperwork before appt as well as schedules for sign in folders and therapist board.

· Obtains picture for chart, pharmacy name, and copies insurance cards for all outside referrals

· Processes payments via credit card machine when necessary

· Enters insurance and demographic information

· Receives incoming PT referrals (inside and outside of our facility). Ensures that each referral contains updated demographics, a legal order with DX code and doctor’s signature, and office or surgery notes when available. Contacts patients to schedule.

· Monitors visit limits and expiration dates for all therapy patients and requests additional visits for patients when needed (i.e. Amerigroup, Humana, Work comp, etc.)

· Creates an APTA chart/ visit for each patient scheduling or rescheduling for therapy.

· Maintains a file of new patient paperwork and orders. Periodically discards out of date orders/ referrals that CX or NS. Scans in all paperwork before discarded.

· Schedules and verifies coverage for urgent same day custom splint patients before or during checkout

· Enters diagnosis description and visit information into APTA

PERFORMANCE REQUIREMENTS:

Knowledge:

· Knowledge of clinic/ organization policies, procedures, systems, and objectives.

· Knowledge of computer systems and applications.

· Knowledge of Optim email and phone systems.

Skills:

· Skills in attention to detail, customer-service orientation, and problem-solving.

· Skills in computer software for scheduling, coding, billing, and overall department function.

· Skills in customer service relations.

Abilities:

EDUCATION: High School Diploma or GED
