Case Manager-Jail
- Salary Range: $24.04 - $28.85/hr based on relevant years of experience
- Location: Larimer county, Colorado
- Status: full time, 40 hours per week
Role Overview:
The Jail Based Behavioral Services (JBBS) program isto develop and provide high quality case management and clinical services. TheCase Manager for JBBS provides direct case management, clinical services, andsupportive services to individuals in Larimer County Jail and in the community,who are experiencing behavioral health issues, including mental illness and/orsubstance use disorders. This role is critical in bridging the gap betweenincarceration and community-based care, supporting treatment engagement,discharge planning, and successful reintegration. This position will alsosupport The DUI Treatment by providing case management, referral / stakeholdercoordination and other client needs.
Essential Duties:
- Provide direct case management services, including referrals, coordination of care, advocacy, and linkage to community resources for the JBBS and DUI Programs.
- Collaborate with jail staff, probation/parole officers, social workers, treatment providers, and family members as appropriate.
- Support clients with reentry planning, including securing housing, mental health treatment, substance use services, medical care, and benefits enrollment.
- Assist clients in improving their well-being and quality of life by coaching in behavioral skills; educate consumers to make informed choices; develop clients coping skills; help clients to solve problems and negotiate tasks of developing a mindful lifestyle.
- Facilitate groups
- Participate in multidisciplinary team meetings, case conferences, and planning sessions.
- Educate clients on life skills, self-advocacy, and community resources to reduce recidivism.
- Respond to crisis situations and provide de-escalation support as needed.
- Documents progress reports, case management notes, letters, faxes and phone calls. Maintains all required paperwork for payee ships.
- Maintain close and cooperative relationships with community agencies and other public agencies and private business in carrying out supportive services.
- Maintain compliance with HIPPA, jail protocols, and agency policies.
- Perform duties as assigned.
Required:
- Relevant bachelor’s degree (i.e. social work, human services, or related field)
- Applicant must be able to have an applicable degree to be on the CAS Track. This would entail ongoing supervision and CAS Trainings.
- 1-3 years minimum of directly relevant experience in the Mental Health Field, Substance Use Disorder Field
- Experience with mild to severe mental illness and substance use disorder
- Experience in coordinating services with other agencies
Preferred:
- CAS preferred
- Bi-lingual / Bi-cultural
Thisposition will include driving a company vehicle and require a valid Coloradodriver’s license
