Progressive Life Center, Inc. (PLC) provides culturally-sensitivity human services to children, youth and families in the District of Columbia and surrounding Maryland, throughout Greater Philadelphia, and in most of Delaware. PLC’s array of community-based services includes foster care and adoption, independent living for youth aging out of foster care, family preservation, juvenile diversion, and parenting skills training. PROGRAM OVERVIEW
Since 1990, PLC Delaware has effectively provided statewide specialized and treatment foster care services, more recently enhancing its programming to provide independent living supports to youth aging out of care, a hotline for foster parents facing crisis (After Hours Crisis Support) and supports for institutionalized foster youth (Parent Partner Program). Through our specialized and treatment foster care, PLC serves children from birth through youth age 23. Intervention focuses on mental health and behavioral issues, permanence and placement stability, and preparation for independence. Hallmarks of the program include use of PLC’s NTU™ Service Delivery Model, skills building group activities, extensive work with birth and foster parents, stakeholder engagement and community involvement, and professional team approach.
POSITION RESPONSIBILITIES
- Provide case management, treatment planning, direct individual services, and documentation of these services for all youth.
- Maintain an electronic client record system of contracts, assessments, and case documentation.
- Encourage motivation, inspire learning, and expose youth to life potential and goals.
- Provide client advocacy and follow-up/collaborate with community providers.
- Complete Life Skills assessments, and Independent Living Plans and updates for all clients.
- Complete documentation for ASSIST stipends, financial literacy, housing, and service exit, as applicable.
- Participate in IL skill building classes, Illumination field trips and events, Youth Advisory Council meetings, and Independent Living coordinators meetings and trainings as scheduled.
- Maintain a caseload of no more than 25 youth.
- Attend and participate in meetings and training.
- Submit accurate documentation in a timely manner.
- Utilize Afrocentric approaches for all interventions.
- Maintain current and comprehensive clinical case files and reports for each child/youth.
- Provide HOPE leadership/advocacy training for ICAN youth.
- Enter and maintain accurate data into the PLC and other required database system(s).
- Complete services to meet 35 billable hours per week.
- Other duties as assigned.
QUALIFICATIONS
- Bachelor’s degree in social work or related field from an accredited college or university.
- Experience in providing services to at-risk youth and young adults between the ages of 16 and 21.
- Knowledge of and experience with Afrocentric or other spiritually focused models or a strengths perspective is desired.
- Demonstrated ability and enthusiasm for working collaboratively.
- Evidence of effective written and verbal communication.
- Experience utilizing MS Office Suite and databases.
- Ability to travel within New Castle County, and occasional other areas throughout the state.
- Owner of a reliable vehicle and valid driver’s license.
- Certificate in NTU Service Delivery Model within one (1) year of hire.
- Ability to work some evening and weekend hours.

PHYSICAL DEMANDS

- Ability to frequently move files weighing up to 20 pounds throughout the office space.
- Ability to be on call and often respond to situations during off hours which can disrupt normal sleep patterns and cause fatigue.
- Ability to face a significant amount of emotional stress which can cause related physical stress on muscles as well as other physical symptoms such as headaches and fatigue.
- In cases of relocation, the incumbent may be required to assist the client by carrying belongings.
- Ability to lift, carry and/or comfort small children.
WORK ENVIRONMENT
Normal office environment, but also may travel locally to visit clients within their homes, meet with service providers, and/or attend meetings: schools, hospitals, mental health clinics, private practices, occasional detention centers/prisons, corporations, and numerous public and private agencies and community settings. Most client service primarily occurs in New Castle County.
