In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.
Program Worker - Casual
Core Function:
Under the overall direction of the Program Manager/Program Coordinator, plans or participates in the planning of appropriate activities and providing high quality services to clients of the programs. Programs within Community Support Services department may include Adult Day Program, Congregate Dining Program, Active Seniors Program, and other related programs within CSS.
Qualifications:
Diploma in social work or social services, health sciences, activation, or equivalent
Minimum 2 years’ experience in a community/social service work setting serving seniors
Knowledge of the social welfare system, social service provision for seniors in the community
Completion of a C.P.R.
Satisfactory Police Check (with vulnerable sector screening) result
Ontario driving license and access to an automobile an asset
Good oral and written communication skills in English and Chinese
Responsibilities:
To coordinate, organize/plan, implement, and evaluate high-quality program activities and services, including development/implementation and review of care plans for clients, and monthly program schedules; in consultation with the Program Manager/Program Coordinator
Coaches and provides guidance to program teams and placement students including daily program plan and scheduling
Provides timely and appropriate supportive counselling, crisis intervention or behaviour management during programs
Delivers personal care to promote independence in clients' daily activities, while respecting clients' rights and choices, and acknowledging the need for dignity and privacy
Assist clients in the proper use and care of adaptive/prosthetic devices
Maintains effective working relationships with clients, caregivers and significant others to provide timely and appropriate support/information and referral to ensure their social/recreational/development needs are appropriately met
Coordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to volunteers
Promotes and publicizes the program to the community
Coordinates and participates in collaborative community partnerships
Complies with organization policies including risk management, health and safety, and infection control procedures
Maintains up to date and accurate files, service records, client progress notes, and statistics on the program
Collects program fees, and maintains accurate record of inventory, fees and expenditures
Keeps up to date with developments and best practices in the field and renewal of any work-related certifications as required
Other duties as assigned
