Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Patient Programs Team! The Team Leader, Licensed Care Management is responsible for the performance management of the Licensed Care Managers and ensures the delivery of comprehensive, high-quality, patient-centered care management services for Erie Family Health Centers high-risk patients. Key responsibilities facilitate onboarding training, oversight of licensed staff members' performance, 1:1 supervision, approving timecards, and support meeting contractual requirements.

Main Duties & Responsibilities

  • Responsible for piloting proposed workflows and providing feedback Senior Program Coordinator
  • Facilitates onboarding training and participates in interviewing candidates as requested by Senior Program Coordinator
  • Supervises licensed staff members through 1:1 supervision, conducts formal evaluations, and performance improvement plans
  • Responsible for interviewing and coordinating training for newly hired and additional coaching/training opportunities based on performance evaluation
  • Provides care management services, including completion of comprehensive risk assessment, care plans, medication reconciliation, and monthly follow-ups for a panel of patients, consistent with contractual and Erie requirements for quality and service excellence

Qualifications


Education

Required:

  • Nurse: Current RN in Illinois.
  • Behavioral Health: Current Illinois LCSW, LSW, LCPC or LPC licensure and master’s degree required.

Preferred:

  • An advanced degree is a plus.

 

Experience

Required:

  • One (1) year of experience as a licensed professional (see Education section below for credentials)
  • Three (3) years of medical or mental health case management/care coordination, or related work experience

Preferred:

  • Experience with staff supervision and performance management preferred.
  • Experience with performing quality assurance and meeting project metrics.
  • Experience working in a community health setting is preferred.

 

Skills and Knowledge

Required:

  • Comfort using telephone interpretation support required.
  • Computer and data-entry skills
  • Ability to establish and maintain effective working relationships with co-workers, clients, and community partners
  • Ability to work with staff members across all levels of the agency
  • Demonstrates ability to coordinate program performance and execute change when needed

 

Preferred:

  • Bilingual fluency (English/Spanish) preferred (verbal and written) (May be required to assist or work with non-English/Spanish only speaking patients and/or guests.)
  • Demonstrates efficiency promoting process improvement
  • Ability to plan, organize, implement, evaluate, and modify program needs
  • Ability to problem solve and address issues as they arise
  • Willing to learn to manage direct reports and people management
