Summary
The Population Health Pharmacy Coordinator will support the implementation and maintenance of medication value-based care initiatives, focusing on improving patient health outcomes. This role involves coordinating medication management activities with pharmacist oversight, ensuring compliance with value-based care workflows, and assisting with data collection and reporting.
Job Duties
- Manage and analyze information from medical records, Excel spreadsheets, and other technology-based systems to support the performance of quality measures
- Provide direct patient outreach to assist pharmacists in confirming insurance information and complete medication histories for patients, monitor disease and medication management in alignment with value-based care workflows, and provide tools and support to address barriers
- Focus on improving medication adherence among patients, particularly in areas such as diabetes, hypertension, and hyperlipidemia, and collaborate with pharmacists to implement changes that enhance the quality of patient care
- Efficiently screen patient profiles in the electronic health record (EHR) and population health reports to identify high-risk patients who may benefit from clinical pharmacist-provided intervention based on established criteria for comprehensive or targeted medication reviews with respect to gaps in care.
- Prioritizes workload, maintains organization, and coordinates activities with other team members.
- Access and update EHR to support accurate medication documentation and communication among healthcare providers
- Communicate with pharmacies and insurance companies to resolve issues related to medication access, such as prior authorizations and formulary restrictions
- Collaborate with physicians, pharmacy team, and other healthcare providers to ensure coordinated and comprehensive care for patients
- Communicates all drug information requests to a pharmacist.
- Contributes to departmental goals and objectives. Performance metrics will be tracked on a monthly, quarterly, and yearly basis, with a focus on payer contract quality ratings such as the Medicare STAR program
- Ensure that all medication management activities comply with regulatory requirements and best practices
Minimum Qualifications
- High School Diploma/GED
- 2 years Pharmacy technician experience or pharmacy intern experience or
- 2 years Certified Pharmacy Technician (CPhT)
- Excellent Interpersonal skills in person and over the phone
- Excellent organizational skills
- Excellent time-management skills
- Familiarity/ability to learn with Microsoft Excel and Third-party data entry platforms
- Basic Microsoft Application and documentation skills
- Knowledge of brand name/generic medications
- Knowledge of medical terminology
- CPhT - Certified Pharmacy Technician - Pharmacy Technician Certification Board Upon Hire
Preferred Qualifications
- Associate’s Degree or
- Bachelor’s Degree
- 3 years Pharmacy technician experience or pharmacy intern experience or
- 3 years Certified Pharmacy Technician (CPhT)
- Work experience as medication reconciliation technician or a role in ambulatory care
- Knowledge and understanding of pharmaceutical and medical terminology and understanding of prescription requirements
- Advanced skills in Microsoft Excel
- Good organizational skills and ability to prioritize tasks
- Proficient with common office resources
Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
