Major Responsibilities:

  • Coordinates relevant logistics for patients moving from acute care or inpatient rehab to outpatient services, both virtual and in person.
  • Supports the clinical and outpatient office teams as a resource assisting multiple rehab teams/locations with complex issues, may delegate tasks across peer team, assists management with employee guidance, and provides performance feedback.
  • Manages the EPIC Scheduled Order Work Queue, following departmental processes for transferring, deferring, or removing orders. Keeps up to date with Medicare, Medicaid, and third-party payer requirements and policies.
  • Supports therapy providers, physicians, key staff, patients, and families to ensure outpatient therapy services are secured at the location with appropriate provider and all other logistical considerations have been made. Insurance and benefit verification and identification of communication resource for services, etc, (eg. Transportation) during acute care or inpatient rehab stays.
  • Organizes patient appointments using established guidelines, recognizes and addresses exceptional scheduling situations, and completes necessary EPIC questionnaires and processes.
  • Obtaining authorizations and physician orders. Collects and verifies medical, social, demographic, and insurance information from hospital records, etc.
  • Educates patients, families, referral sources, hospital staff, external payers, and other stakeholders about rehabilitation therapy benefits and services. Maintains good public relations and communicates customer service and process improvement issues to management.

Licensure, Registration, and/or Certification Required:

  • None

Education Required:

  • Associate degree

Experience Required:

  • 3-5 years experience in a complex medical environment.

Knowledge, Skills & Abilities Required:

  • Solid knowledge of various insurance coverages. Proven ability to network and problem solve with internal and external stakeholders. Solid understanding of healthcare industry and pre-service/insurance applications.

Pay Range

$24.85 - $37.30
