About This Role
This is an exceptional opportunity for a mission-driven, innovative clinician who is passionate about reimagining how care is delivered to older adults. As the Director of Clinical Program Design, you’ll play a key role in shaping and evolving our care model to ensure it reflects what matters most to our participants—delivering high-quality, person-centered care that supports independence, dignity, and well-being.

In this role, you’ll report to the Vice President and National Medical Director and collaborate closely with teams across operations, learning and development, product and technology, and beyond. You’ll lead the design of forward-thinking programs—such as virtual care, home-based care strategies, behavioral health integration, and other emerging innovations—while partnering with operational leaders to bring those ideas to life.

Whether you’re creating new clinical workflows, supporting education and training, or helping scale care delivery in new markets, your work will be rooted in compassion, curiosity, and a deep commitment to improving the lives of high-risk seniors. This is more than a clinical leadership role—it’s a chance to build the future of elder care, one thoughtful program at a time.
