Job Summary: The Caribbean VA Healthcare System's Geriatrics and Extended Care Service is seeking a Board Certified Geriatrics Physician to serve as the Home Based Primary Care (HBPC) Medical Director at the San Juan Department of Veterans Affairs Medical Center. The HBPC Medical Director participates in the creation of policies, procedures and guidelines to assure provision of adequate good quality comprehensive patient care in the HBPC program.
Major Duties:
- Duties of the Home Based Primary Care (HBPC) Medical Director include, but are not limited to: The HBPC Medical Director participates in administrative decision making and recommends and approves relevant HBPC policies and procedures. The HBPC Medical Director in collaboration with the HBPC Chief Nurse advises the HBPC and medical center about the appropriateness of admissions and discharge criteria, related orders, in the HBPC program. The HBPC Medical Director is responsible for collaboration with other HBPC providers who represent expertise in specialty areas such as but not limited to Mental Health, and Physiatrist (PMR) and to provide liaison between HBPC team and the VA facilities physicians and coordinate care with other physicians/subspecialties throughout the Medical Center. Assess the patient's conditions prior to hospital discharge as needed or during the first month in HBPC. Advices the ACOS for Extended Care about adequacy and appropriateness of the HBPC scope of service for patients, medical equipment and its professional staff. Provides input and recommendation to Service Chief for credentialing and privileging of all HBPC providers. The HBPC Medical Director participates in family meetings and similar activities as appropriate, to assist in promoting and protecting patients' rights. The HBPC Medical Director collaborates with the interdisciplinary team, Veterans, their surrogates, and allied services within and outside of the organization to encourage planning, implementing, and evaluating clinical services that maximize resident choice, quality of life, and quality of care. The HBPC Medical Director is responsible for ensuring that the Focused Professional Practice Evaluations and Ongoing Professional Practice Evaluations (FPPE/OPPE) are completed on all providers as required by policy. Participates with the Chief Nurse for HBPC in establishing staff, house staff, and employee's in-service training's. Participate in the academic activities of the Service and in resident's supervision. VA offers a comprehensive total rewards package. VHA Physician Total Rewards. Qualifications: To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty Geriatrics Training Program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Board Certified in Geriatrics. Selective Placement Factor (SPF). This position includes a skill, knowledge, ability, or other worker characteristic basic to -and essential for- satisfactory performance of the job. This position requires: light to moderate lifting (15 - 44 lbs.); light carrying (15 lbs. and under); reaching above shoulder; use of fingers; both hands required; walking (up to 2 hours); standing (up to 2 hours); kneeling (up to 1 hour); ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16"; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish basic colors; ability to distinguish shades of colors; hearing (aid permitted); emotional stability; mental stability; working closely with others; and working alone.
