Job Description:
The Stroke Nurse Navigator plays a pivotal role in ensuring the delivery of high-quality, evidence-based care for patients diagnosed with stroke, stroke-like symptoms, and/or TIA (Transient Ischemic Attack). This role involves close collaboration with multidisciplinary teams, including medical and hospital staff, therapists, and the Stroke Activation Team, to provide education, support, and coordination of care. The Stroke Nurse Navigator also ensures compliance with regulatory standards and assists in performance improvement efforts, ultimately striving to enhance patient outcomes and experience throughout their stroke care journey.
Key Responsibilities:
1. Chair of Gloucester Stroke Club Support
2. Inpatient Rounds:
o Perform daily rounds as needed on inpatients with a primary diagnosis of stroke, stroke-like symptoms, and/or TIA (transient ischemic attack), including patients undergoing stroke workup.
3. Patient Reviews:
o Review all incoming stroke patients to ensure compliance with The Joint Commission (TJC), American Heart Association (AHA), Get With The Guidelines (GWTG), and other regulatory standards.
4. Regulatory Compliance:
o Support the stroke program in maintaining compliance with state and national standards for designated stroke centers.
5. Multidisciplinary Collaboration:
o Collaborate with the medical team and hospital staff to ensure timely, high-quality, and multidisciplinary care for stroke patients.
6. Program Development:
o Work with the Stroke Program Manager and Stroke Medical Director to develop program plans, policies, protocols, and care pathways that support effective and timely care for stroke patients.
7. Data Collection and Analysis:
o Collect and analyze stroke program patient data to support documentation, improve clinical care, and enhance patient outcomes.
8. Education and Training:
o Develop educational materials and resources for staff, patients, and the community to promote stroke awareness and education.
o Provide education to staff and patients, ensuring they understand stroke care processes and prevention strategies.
9. Performance Improvement:
o Support the development and coordination of performance improvement teams, ensuring stroke-related protocols and therapies are adopted as standard care within the facility.
10. Stroke Activation Team:
o Supports the Stroke Activation Team, providing guidance on stroke care processes and workflows.
11. Patient Education:
o Serve as a leader in stroke education, working with bedside nurses, therapists (PT, OT, SLP), and the neurology team to provide stroke education to patients and families.
12. Caregiver Readiness:
o In collaboration with case management, physical therapy (PT), and occupational therapy (OT), assess caregiver readiness to support the patient post-discharge.
13. Family Support:
o Educate and counsel patients and their family/support networks on stroke care, recovery, and available services.
14. Registry Data Review:
o Collaborate with the Stroke Program Manager to review registry data for fallouts, discrepancies, and inaccurate coding.
15. Additional Duties:
o Perform other duties as necessary, based on the needs of the stroke program or hospital.
Qualifications:
· Registered Nurse (RN) with current licensure.
· Experience in stroke care or related medical/surgical nursing required.
· Strong clinical knowledge of stroke care protocols, regulatory guidelines, and performance improvement processes.
· Excellent communication, teamwork, and leadership skills.
· Ability to educate patients, families, and staff on stroke-related issues and services.
