The Pediatric Transitional Care Specialist provides support and guidance for pediatric patients with a chronic disease or condition diagnosis This position coordinates referrals from physicians and health care facilities, acts as a liaison between patient and physician practices, participates in multidisciplinary care conferences, offers patients and their families psychosocial support and access to resources, and educates on disease specifics and treatment options. In addition the position works with designated physicians to develop and clinical pathways and plans of care. Employees providing direct patient care must demonstrate competencies specific to the population served.
Bachelor's of Science in Nursing required 5 years of professional nursing experience required Experience in pediatric or neonatal nursing is preferred. Licensed Registered Nurse in good standing with the State of Michigan
Must provide evidence of successful completion (or an approved audit) of a CPR training course within the past two years or successfully complete a CPR training course (or audit with approval) within six months of hire. • Must possess excellent interpersonal, effective problem solving and prioritization skills, and the ability to process information in a confidential manner • Must be able to communicate clearly and effectively with patients, providers and other staff • Has the ability to maintain patient confidentiality, work independently and think as well as function as an integral team member with other healthcare professionals • Must also possess excellent computer skills and have ability to read, interpret, and analyze data from various computer systems and equipment • Must have excellent verbal, written and electronic communication skills • Has the ability to maintain positive customer service • Has the ability to establish effective working relations with patients, staff and physicians in a team environment Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time. The job produces some physical demands. Typical of jobs that include regular walking, standing, stooping, bending, sitting, and some lifting of light weight objects. • Works closely and acts as a liaison with General Pediatricians and office staff between many specialty areas including but not limited to Hem/Oncology, Neurology, Cardiology, Neonatology, Outpatient Testing, Surgery and Outpatient Treatment facilities to coordinate referrals of patients with in Bronson and outside practice. • Provides support to coordinate appointments and care for patients and their families. Provides psychosocial support and access to resources for patient and family. • Participates in Multidisciplinary Care conferences. Advocates for patient. Provides interpretation of potential treatments for patient and family. • Works with designated physicians and staff to develop plans of care and maintain clinical pathways. Tracks interventions and outcomes. • Under the direction of the provider, provides patient and family with appropriate education and reinforcement teaching regarding their care. • Assist in scheduling tests, diagnostics, and procedures for patients as needed. • Tracks volume, clinical outcomes, patient and physician satisfaction. • Assists families in accessing community resources. • Proactively develops the role of Nurse Transitional specialist with the goals of improving the experience for referred patients and the referring physicians. • Assures proper communication back to referring physicians regarding care and treatment of patients. • Attends staff, educational, and other meetings as required. Participates on hospital committees as assigned. • Familiar with various insurance and referral policies. • Acts as liaison to Palliative care as appropriate.
