SUMMARY OF DUTIES
In this position, you will:
Assist case managers in the delivery of long-term care and financial benefits to seniors and people with disabilities. Conduct make home visits on a regular basis for various reason which might include ensuring the development of service plans is effective, obtain and assist Oregonians with documentation needs for their long-term care services and financial benefits. Perform as an adjunct to case managers and must utilize judgement in recognizing and assessing client’s health and functional status and be knowledgeable of issues representing potential abuse and neglect. Process and enter authorized payments for various services such as homecare worker payments, Meals on Wheels, Emergency Response Systems. Have direct contact with providers and Oregonians related to their long-term care benefits. This position requires accurate computer data entry of case management information.
MINIMUM QUALIFICATIONS
One year of experience assisting professionals in one or more of the following specialized mental health/social /human services program areas:
Economically disadvantaged
Substance dependent
Mentally or physically disabled
Child abuse and neglect
Juvenile delinquent
Any degree in a related mental health or human services program may substitute for six months of the required experience.
ESSENTIAL ATTRIBUTES
We are looking for candidates with:
1- Daily work maintaining the confidentiality of sensitive information including financial and PHI in a business office setting.
2-Advanced computer skills including creating spreadsheets with tables, graphs, & formulas in a business office setting.
3-Work experience organizing and managing multiple tasks with frequent interruptions in an office setting
4-Experience working in a team including planning, goal setting and covering other worker's duties in a business office setting

WORKING CONDITIONS
Majority of work is performed in an open space office environment. However, frequent out-of-office tasks and client home visits may be required. Must possess and maintain a driver's license and an acceptable driving record.
