Position Duties
GENERAL PURPOSE
The Admissions Coordinator position is responsible for all aspects of the Gracedale Nursing Home admissions process, marketing, and the management of referrals. This position works toward increasing the nursing home census and strives to promote/foster a good reputation on behalf of the facility throughout the community.
SUPERVISION RECEIVED
This position reports directly to the Nursing Home Administrator.
SUPERVISION EXERCISED
This position is responsible for supervision over the following classifications: Clinical Liaison and Secretary 2 – Social Services.
ESSENTIAL DUTIES OF THE POSITION
Records/logs all referrals. Communicates with all referrals to ensure successful admission to Gracedale.
Responsible for assimilation, tours, clinical evaluations, payment coverage confirmations, and obtaining signatures on contracts, consent forms, and other authorizations.
Coordinates transfer of care with continuity.
Designs marketing plans. Markets the facility and services to the referral community. Collects market place feedback to provide to management and for utilization in the development of new or expanded services. Chairs the marketing committee.
Represents Gracedale Nursing Home and services provided at community events.
Establishes payer type for each admission and works with the Business Office to ensure proper billing and coverage.
Tracks customer flow records for benchmark and performance evaluation. Prepares monthly report on department activity and results.
Communicates regularly with all referral sources.
Tracks bed availability including bed holds (paid and unpaid) and offers beds as available and appropriate.
Supervises Clinical Liaison and works closely in accomplishing departmental and Gracedale goals.
Works with the Nursing Home Administrator to interface with all Gracedale departments to ensure successful and satisfactory admission processes, continuity of care, and quality services. Provides customer service training to new hires and as needed or requested by Nursing Home Administrator.

REQUIRED MINIMUM QUALIFICATIONS
EDUCATION & EXPERIENCE: Graduation from an accredited college or university with a Bachelor's degree in Social Work or other human services field such as Sociology, Special Education, Rehabilitation, Counseling or Psychology; AND
At least three (3) years of full-time professional work experience in a health care facility, preferably in long term care. Clinical and/or nursing experience is preferred.
All employees of Gracedale Nursing Home will be required to attend Feeding Program training.
KNOWLEDGE, SKILLS, AND ABILITIES
Thorough knowledge of the methods and techniques used in gathering and analyzing information pertaining to the admission process, discharge, and eligibility for reimbursement for services in a long-term care facility.
Knowledge of Medicare, Medicaid, and private insurance rules and procedures with an understanding of payer types and what coverage(s) are available for long term care.
Knowledge of management/supervisory techniques including the ability to plan, assign, and review the work of subordinate employees.
Knowledge of sales and marketing strategies required in long-term care.
Skill in eliciting information from, and/or providing explanations to, patients and their families on personally sensitive or confidential issues.
Ability to establish and maintain effective working relationships with associates, patients, their families, representatives of other social service agencies, and other facility personnel.
Ability to operate a personal computer and secure information from established data processing, spreadsheet, word processing, database, and graphics programs.
Ability to read, write, speak, understand, and communicate in English to perform the duties of this position.

Telephone, personal computer (including word processing and spreadsheet software), calculator, fax machine, copy machine, and paper shredder.
