Position Summary

The Behavioral Health Community Liaison is responsible for strengthening Nexus Health Systems’ connections with referral partners, child welfare organizations, payers, and community stakeholders throughout Texas. Additionally, the position will support local community relationships for our San Antonio Residential Treatment Program. This role develops and nurtures referral pathways for all Nexus programs, including behavioral health, serious injury and chronic illness rehabilitation, and specialized spinal and brain injury services.

This individual serves as a trusted resource in the community, drawing on behavioral health expertise and established networks in child welfare and among referral partners and payers to ensure families and providers understand the unique programs Nexus offers.

Key Responsibilities

  • Community & Referral Development
    • Build and maintain strong relationships with child welfare organizations, behavioral health providers, case managers, and payers.
    • Act as the primary liaison for referral sources in San Antonio, while extending support across Texas as needed.
    • Develop referral pathways for all Nexus programs, ensuring broad awareness of services.
  • Strategic Engagement
    • Represent Nexus at community events, child welfare forums, and professional conferences.
    • Conduct needs assessments with referral partners to identify gaps and match patients with appropriate Nexus programs.
    • Provide tours, presentations, and educational sessions to increase awareness and trust in Nexus services.
  • Clinical Collaboration
    • Collaborate with clinical and program leadership to ensure smooth transitions for referred patients.
    • Maintain working knowledge of admission criteria for all Nexus programs to guide referral discussions.
    • Provide initial screenings or assessments, as appropriate, to support timely admissions.
  • Operational Excellence
    • Accurately document referral activity and maintain current data in CRM systems.
    • Contribute insights to growth strategies and program development based on community feedback.
    • Ensure all outreach activities align with compliance standards and ethical practices.

Qualifications

  • Education:
    • Bachelor’s degree required; advanced degree in behavioral health or social services preferred.
  • Experience:
    • Minimum three (3) years of experience in behavioral health, child welfare, or related healthcare field.
    • Proven background in referral development, case management, or community partnerships.
    • Familiarity with Texas child welfare systems, medical providers, and community-based behavioral health resources strongly preferred.
  • Skills & Competencies:
    • Excellent communication, relationship-building, and networking skills.
    • Ability to engage diverse stakeholders, from child welfare professionals to healthcare providers.
    • Strong organizational and problem-solving skills with attention to detail.
