Job Purpose

As a Specialist Service Coordinator you will work close with NHS to coordinate and manage patients and professionals through defined pathways to enable patients to see the right person, at the right time and in the right place. You will provide a central point for all referrals/information/advice for Adult and Paediatric Critical Care.

Key Responsibilities

  • Act as a point of contact for incoming referrals, communicating with all relevant personnel in the process.
  • Receiving and dealing with calls, e-referrals, emails and queries from patients and professionals and handling these as required, in order to coordinate patient care according to agreed pathways (urgent & non-urgent).
  • Organise and coordinate a range of activities between different organisations to prevent acute admission and to facilitate discharge in line with agreed protocols
  • Making outbound calls to patients and professionals in order to coordinate patient care according to agreed pathways and service agreements.
  • Establish and maintain effective communication with various individuals and groups.
  • Ensure that data is kept secure and maintain the confidentiality of staff and patients at all times.
  • Contribute to the databases for patients, updating records and contributing to the collection of data for audit and research purposes.
  • Working flexible shifts over 7 days, 365 days a year


Experience/Knowledge/Competencies You Will Need

  • Previous experience working in contact centre
  • Exceptional communication skills
  • Adept at conveying complex and sensitive information with poise and clarity, even in high-pressure and demanding environments.
  • Advanced listening and facilitative skills across a diverse range of calls, some of which will be highly challenging and emotive.
  • High levels of concentration when making and receiving calls whilst accurately recording outcomes onto the computer.
