We are looking for an experienced and motivated Licensed Practical Nurse (LPN) to join our growing team in East Haven, CT!

About Us: Utopia Home Care is a Medicare certified, state licensed, medical home care agency that provides in-home nursing, therapy, and home health aide services throughout Connecticut.  We have been family owned and operated since 1983 and currently serve over 1400 clients throughout the state!

Licensed Practical Nurse (LPN) Qualifications / Requirements:

• Must have an LPN certification and possess the original document of certification from a state approved training provider.
• Must be able to provide original documents for CT Driver's License or CT State ID and original Social Security Card.
• Must be able to provide documentation of a physical and PPD taken within the past 12 months OR be willing to obtain a physical and PPD test quickly.
• Previous experience in homecare is a must.

Licensed Practical Nurse (LPN) Job Duties: LPNS can provide direct nursing services which include periodic health assessments, teaching, skilled intervention, care coordination, and medication management / medication administration. All of this takes place in the client's home! We are seeking LPN's for med admin evening runs specifically!

Utopia provides a full spectrum of nursing services such as Medical / Surgical, Wound Care, Orthopedic Care, and Behavioral Health. Qualifications

LPN Requirements:

• Must be currently licensed in the State as a Behavioral Health Nurse, preferably with one (1) year of experience. 
• Experience with behavioral health required.
• Previous experience with Medicare/OASIS required.
• Assist with on call rotations (paid) and weekend SOC's/ROC's
• Possess good communication and organizational skills.
