REQUIREMENTS:
- 2 Years of experience as a supervisor in a hospital, nursing home, or other related health care facility.
- Current CNA License or the ability to get one. (Tuition Assistance provided)
- Knowledgeable of nursing and medical practices, procedures, laws, regulations and guidelines pertaining to long-term care.
- Current BLS/CPR Card

SUMMARY
The Resident Care Coordinator (RCC/CNAs) coordinates, supervises, and evaluates the care of residents in both skilled and long-
term care. The RCC is responsible for supervising floor staff (Caregivers, Med Techs) to ensure clinical services are delivered in
accordance with regulations.
ESSENTIAL FUNCTIONS
. Participate in the orientation of new residents/family members to the facility
. Admit and discharge residents with documentation regarding care and service provided
. Communicate directly with residents, medical and nursing staff, family members, department heads and members of
the interdisciplinary team to coordinate care and services and respond to and resolve complaints and concerns in a
timely manner
. Assist in oversight of resident incidents and concerns daily to identify any unusual occurrences and reports them
promptly to the Director of Nursing, Administrator, and/or state agency for appropriate action
. Complete daily clinical and environmental rounds to observe residents/patients and ensure their needs are being met
. Maintain a current reference of written nursing material to assist the nursing service department meet the residents'
day-to-day needs
. Update Care plans
. Complete MDS'
. Recommend the equipment needs of the nursing department to the Director of Nursing
. Ensure that all nursing personnel comply with the written policies and procedures
. Follow clinical programs, including risk management, infection prevention, quality, and staff development
. Work collaboratively with other members of the team to promote and role model teamwork, mutual respect and
effective communication
. Participate in daily or weekly management team meetings to discuss census changes, resident changes in status,
complaints, or concerns
. Participate in QAPI or facility assessment activities as needed, such as carrying out duties assigned as part of a
performance improvement committee
. Facilitate, serve, attend, and/or participate in various committees of the facility as appointed
. Conduct observations of nursing care and supervise development of in-service education to ensure nursing staff is
competent in current knowledge and skills
. Participates in the facility’s plan of correction response to an inspection survey and implements any follow-up
required for nursing allegations
. Monitors for allegations of potential abuse or neglect, or misappropriation of resident property and participates in the
investigative process
. Promotes safe work practices, safety rules, and accident prevention procedures to prevent employee injury and
illness
. May provide direct nursing care as necessary
