FACT Care Coordinator

Hiring Range: $26.94 - $32.55 Per Hour (Includes FACT Differential)

Kitsap Mental Health Services (KMHS) is hiring a Care Coordinator to join our Forensic Assertive Community Treatment (FACT) team. FACT provides time-limited services to clients most impacted by serious mental health conditions such as schizophrenia and schizoaffective disorders. FACT teams carry small caseloads to provide individualized support, with most services delivered in homes and community settings. As a Care Coordinator, you will provide comprehensive case management, coordinate services across multiple domains, and support clients in achieving stability, recovery, and independence.



Primary Responsibilities:

  • Provide active case management to address housing, health and hygiene, finances, food sufficiency, legal requirements, social skills, recreation, and activities of daily living.
  • Participate in individualized treatment planning, ensuring plans reflect recovery-oriented goals and client input.
  • Use recovery-focused language and clinical strategies to help clients strengthen independence and quality of life.
  • Engage and motivate clients to make behavioral changes that support long-term recovery.
  • Provide psychoeducation to clients and families regarding diagnoses, treatment, and recovery from psychotic disorders.
  • Document progress and treatment activities in a timely, accurate manner that connects case management to treatment goals.
  • Participate in daily staff meetings and scheduled treatment plan reviews.
  • Support clients in reducing or abstaining from substance use, providing sober coping skills and linking to KMHS co-occurring treatment when needed.
  • Apply situational awareness to evaluate safety and reduce risks in client homes and community settings.
  • Collaborate with Mobile Crisis Outreach Team, law enforcement, and other partners during crisis intervention as directed by supervisor.
  • Coach clients in independent living skills such as shopping, cooking, cleaning, budgeting, transportation, and socialization.
  • Serve as primary clinician for up to 25 clients, managing treatment planning and documentation.

 

Minimum Qualifications:

EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.

EXPERIENCE:

                      Entry level (no prior related work experience)

                      This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation.

LICENSURE: Agency Affiliated Counselor Registration.

 

Preferred Qualifications:

EXPERIENCE:

                      Experienced (minimum 2 years of job-related experience)

                      Experience providing comprehensive case management to clients in a community mental health or social services setting.

 

Performance Requirements:

KNOWLEDGE:

  • Maintain a working knowledge of all KMHS facilities and clients resources, as well as community resources.
  • Maintain a working knowledge of FACT treatment standards for psychiatric rehabilitation.

 

SKILLS:

  • Skill in establishing and maintaining effective working relationships with other employees, clients, organizations, and the public.
  • Communication   Communicates clearly and concisely.
  • Computer skills   Ability to operate a personal computer, fax machine, printer, and copier proficiently.

 

ABILITIES:

  • Demonstrate ability to meet or exceed productivity standards.
  • Demonstrate ability to work cooperatively and collaboratively as a team member.
  • Demonstrate ability to take direction from supervisor in fluid clinical scenarios to meet client needs, in coordination with other team members.

REQUIREMENT: Active Driver’s license and acceptable motor vehicle report with reliable, insured transportation.

 

Equipment Operated: Standard office equipment including computers, fax machines, copiers, printers, telephones, cellular telephone, etc. Usage of motor vehicle.

 

Work Environment: Office, outreach.

 

Mental/Physical Requirements:  While performing the job duties, the employee is required to walk, sit, use hands and fingers, reach with arms, talk, or listen.  Peripheral vision is also required for this position. Frequent mobility and/or sitting required for extended periods.
