Essential Duties and Responsibilities

  • Assesses, diagnoses and treats medical and surgical problems in outpatient clinics, patient's homes and nursing homes, and records such findings in the patient chart. Refers patients to appropriate physicians.
  • Elicits medical histories and performs physical examinations on patients.
  • Performs therapeutic, office surgical, diagnostic and laboratory procedures and radiologic examinations as indicated by common medical practice and as delegated by the supervising physician.
  • Instructs and counsels patients regarding physical and mental health and patient education. Explains plan of care and treatments to patients and families to assure understanding.
  • Administers or prescribes treatment and medications as required per standards of care.
  • Maintains current knowledge and competence in the areas of diabetes, health improvement and chronic disease management. Assists with wound care as needed.
  • Coordinates services provided in diabetes, health improvement and chronic disease management.
  • Works as a clinical expert in supporting and educating staff and community through the composition of in-services, presentations, demonstrations, and in the implementation of prevention and treatments for chronic disease management, diabetes, and health improvement.
  • Serves as coordinator for Diabetes ADA Recognition Program, Medicare Diabetes Prevention Program, and CDC Diabetes Prevention Program. Submits required data to maintain certification.
  • Participates in and/or maintains a Performance Improvement project on an ongoing basis, for diabetes, and/or health improvement.
  • Completes documentation in a timely manner.
  • Functions independently in most activities. Provides care for patients. Adheres to hospital /clinic policies and procedures and medical staff rules, regulations and by-laws.
  • Maintains orderly and efficient business practices at the clinics.
  • Provides supervision and direction to other clinic personnel.
  • Respond to emergency medical situations in an urgent manner and perform on the spot emergency assistance which may include first aid and/or CPR.
  • Practices and behaves in a professional, safe and ethical manner at all times.
  • Serves as a patient advocate.
  • Maintains friendly and courteous interpersonal communication skills and a positive service attitude.
  • Participates in performance improvement and educational activities. Completes all CEU’s as required by respective licensure. If licensed, maintains an active and current license.
  • Works willingly and accepts assignments as given.
  • Attends all mandatory meetings and programs.
  • Participates in opportunities that enhance personal and professional growth.
  • Performs other duties as necessary or assigned.
  • Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.
  • Complies with all applicable state and federal regulations and JCAHO standards.
  • Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.
  • Maintains the confidentiality of patient, resident, employee and organizational information.

 

Supervisory Responsibilities

Assume Director’s role during his/ her absence.

Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

May assist with special unit procedures and provide training, education, and guidance to assigned R.N.s, L.P.N.s, C.M.A.s and/or C.N.A.s.

 

Qualifications

Education and/or Experience

  • Successful completion of an approved nurse practitioner or physician assistant program.
  • Because of the nature of responsibilities, it is preferable to have experience in counseling and teaching and have a master's degree.
  • Current approval by the joint Board of Medical Examiners and Board of Nursing to perform medical acts.
  • Must be currently licensed as a Registered Nurse in the state of Iowa.
  • Three years’ experience with emphasis in Chronic Care/Diabetes management experience preferred.

 

Position Requirements, Licenses, Certifications, Registrations

  • Basic Life Support certification.
  • Management of Aggressive Behavior Certification within 6 months of hire.
  • Ability to create a caring, compassionate relationship with the patients and families.
  • Ability to collaborate with the healthcare team to use evidence-based practice in providing care.
  • Expertise in handling sophisticated medical equipment.
  • Must possess effective time management skills.
  • Ability to carry out assignments with minimal supervision and guidance.

 

Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively before customers, patients, families and employees of the organization.

While performing the duties of this job, the employee is regularly required to stand; talk and hear; and use hands to finger, handle or feel.  The employee is occasionally required to stoop, kneel, crouch, and reach with hands and arms.  The employee is frequently required to walk and sit.  The employee must frequently lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.  Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, depth perception and ability to adjust focus. May be exposed to infectious diseases and occupational dangers like accidental needle pricks. Must be able to withstand physical and emotional stress.
