The Community Outreach Coordinator will serve as the local face of Mom’s Meals throughout New York, working directly with healthcare providers, case managers, social service agencies, and individuals to promote and facilitate the growth and access to Mom’s Meals. This role is responsible for driving awareness, building relationships, and supporting program enrollment to help ensure nutritious meals reach those who need them most. This position reports to the Manager of Healthcare Partnerships. The successful candidate will drive an increase in HDM referrals from a variety of organizations throughout New York.
This position will be based in New York, ideally located in Albany, Schenectady, or Troy (with consideration given to nearby regional hubs such as Binghamton or Saratoga Springs).
This role will support Social Care Network (SCN) regions including the Hudson Valley, Capital Region, North Country, Central New York, and Southern Tier. The position will travel regionally and statewide up to 75% and is eligible for a company car.

Position Responsibilities may include, but not limited to
Work closely with SCN Lead Entities, screeners, and navigators to educate, communicate and advocate for nutrition services including nutrition counseling, Medically-Tailored Meals, Prescription Boxes, and Pantry Stocking
Outreach to ECM providers, Federally Qualified Health Centers (FQHCs), hospitals, Long Term Care (LTC) facilities, and Community Health Worker (CHW) organizations to engage and provide overview of MM services
Build and maintain strong relationships with key community stakeholders, including social workers, discharge planners, senior centers, non-profits, and other community locations
Promote Mom’s Meals products and products through in-person outreach, community events, presentations, and educational sessions
Increase awareness of the 1115 Waiver program in New York that allows eligible Medicaid members to access HRSN services including nutrition
Identify and engage eligible clients, assisting them with understanding their options with successfully screening and navigating through the process of benefits covered under additional Medicaid/Medicare and self-pay options
Act as a liaison between clients, referral sources, and internal teams to ensure seamless service delivery
Track outreach activities, client engagement, and outcomes using CRM tools or internal systems
Stay up to date on local and state programs that offer meal benefits and collaborate to align services
Represent Mom’s Meals at local fairs, expos, conferences, and coalition meetings
Provide feedback from the field to improve customer experience and address barriers to access
Offer and schedule onsite (preferred) or virtual in-services with target organizations
Be highly visible within NY communities by engaging in community-based event
Attend and participate in county round tables and and state webinars to represent Mom’s Meals
Cover for Manager of Healthcare Partnerships when needed
Required Skills and Experience
Associate’s or Bachelor’s degree
2 years of community outreach, sales, or health care
Demonstrated ability to successfully engage community health organizations
Proven ability to work with cross functional teams
Excellent oral and written communication skills; experience giving presentations
Experience using Microsoft Office suite, including PowerPoint and Excel
Willingness to travel locally and statewide; travel 75%
Preferred Skills and Experience
Experience in community outreach, sales, social services, or healthcare
Strong interpersonal and communication skills, with a customer-focused approach
Comfortable working independently in the field with minimal supervision
Passion for serving underserved or vulnerable populations
Knowledge of Medicaid, Medicare, and social assistance programs
Must possess a valid driver’s license and reliable transportation
Physical Requirements
Repetitive motions that include the wrists, hands and/or fingers
Sedentary work that primarily involves sitting, remaining in a stationary position for prolonged periods
Visual perception to perform job including peripheral vision, depth perception, and the ability to adjust focus
