Overview

The Outreach Team Member is the first point of contact for individuals who may qualify for Enhanced Care Management (ECM). In this role, you will connect high-need Medi-Cal members to services that promote stability, reduce barriers, and improve health outcomes. This position blends outreach, patient engagement, and advocacy—ideal for someone who is empathetic, resilient, and motivated by mission-driven work.

Responsibilities

1. Outreach & Engagement

  • Initiate outreach through phone, SMS, email, and chat.
  • Prioritize high-need prospects using lead lists while maintaining a personal, human-centered approach.
  • Tailor communication to each individual’s health needs, preferences, and background.
  • Complete assigned outreach campaigns and member enrollment activities.

2. Program Education & Enrollment

  • Clearly and compassionately explain ECM program benefits.
  • Conduct eligibility screenings and assist participants through the enrollment process.
  • Set expectations early and ensure a smooth, supportive onboarding experience.

3. Relationship Building

  • Build trust using motivational interviewing and trauma-informed care principles.
  • Communicate with cultural sensitivity across diverse populations and literacy levels.
  • Identify additional needs (food, housing, behavioral health, etc.) and escalate appropriately to the care coordination team.

4. Collaboration & Teamwork

  • Work closely with internal teams including Member Coordinators, Case Management, Provider Relations, Marketing, and Outreach.
  • Share insights and feedback from outreach interactions to strengthen processes and referral pipelines.
  • Participate in team meetings, coaching sessions, and collaborative initiatives.

5. Reporting & Accountability

  • Document every interaction accurately in designated platforms (CRM, chat tools, etc.).
  • Track personal metrics and participate in weekly check-ins and performance reviews.
  • Maintain strong compliance in documentation, follow-up, and outreach activity completion.

Requirements

Core Skills

  • Strong communication skills (verbal, written, digital).
  • Resilient, goal-oriented mindset with the ability to handle rejection.
  • Ability to adapt tone and approach for different populations (e.g., seniors, caregivers, non-English speakers).

Technical Skills

  • Comfortable using CRMs, HIPAA-compliant chat platforms, and telehealth tools.
  • Ability to work with spreadsheets and reporting dashboards.

Experiential Skills

  • For patient engagement, at least 3-5 years in sales, outreach, and cold calling (or 2-3 years in a healthcare setting).
  • Demonstrated success in meeting enrollment, sales, and goal targets.

Preferred

  • Knowledge of Medi-Cal/Medicare systems
  • Multilingual fluency.
  • Experience working with underserved or high-need populations.

Success Measures

  • 12+ new enrollments per week
  • 72%+ conversion rate from first contact to enrollment
  • 100% outreach activity completion and timely follow-ups
  • 92%+ documentation accuracy across platforms
  • Positive feedback from members and strong collaboration with internal teams
