Description
The Certified Nursing Assistant and Transportation Coordinator role combines the responsibilities of a Therapy Technician and Transportation Coordinator, providing support for therapy services and managing transportation services for residents of the facility.
Schedule: Temporary- Full-Time, Monday-Friday, Day 8-12 hours
Primary Responsibilities and Duties:
CNA Responsibilities:
- Provide direct care to residents under the supervision of licensed nursing staff, including assistance with bathing, dressing, grooming, toileting, and feeding.
- Assist residents with mobility and transfers, using proper lifting and position techniques to ensure their safety and comfort.
- Monitor residents’ vital signs, intake and output, and report any changes or concerns to the nursing team.
- Assist with resident admission, transfers, and discharges, including orienting new residents to their surroundings and helping them settle into their rooms.
- Support residents' social and recreational activities by engaging them in meaningful interactions, escorting them to group activities and providing companionship.
- Maintain a clean and organized living environment for residents, including making beds, tidying rooms, and ensuring cleanliness and infection control standards are met.
- Assist with resident dining services, including meal set-up, feeding assistance, and recording food and fluid intake as directed.
- Document care provided and resident observations accurately and timely in accordance with facility policies and regulatory requirements.
- Participate in educational opportunities and training programs to maintain CNA verification and enhance skills and knowledge in geriatric care.
- Assist new CNA team members with training and getting actuated with policies, procedures and the layout of the facility.
- Participate in team meetings and contribute to resident care planning and goal setting.
- Contribute to a positive and supportive work environment by demonstrating professionalism, teamwork, and effective communication skills.
- Perform all other duties assigned.
Transportation Coordinator Responsibilities:
- Coordinate transportation services for residents to medical appointments, therapies, and other off-site activities.
- Develop and maintain transportation schedules, considering patient needs, staff availability, and vehicle availability.
- Communicate transportation arrangements to patients, families, and staff members in a timely and clear manner.
- Ensure compliance with state and federal regulations governing transportation services for healthcare facilities.
- Maintain accurate records of transportation activities, including trip logs, mileage, and vehicle maintenance records.
- Conduct regular inspections of transportation vehicles to ensure they meet safety standards and address any maintenance issues promptly.
Requirements
Qualifications:
- Valid CNA license in the state of Arizona
- Prior experience in a skilled nursing facility or similar medical setting preferred.
- Ability to build rapport with residents through a compassionate and caring demeanor.
- Commitment to providing high-quality, resident-centered care with empathy and compassion.
- Strong clinical skills in assessment, treatment planning and intervention implementation.
- Strong communication, interpersonal, and organizational skills.
- Valid driver's license with a driving record that meets company policy requirements.
- Familiarity with transportation regulations and safety standards in healthcare facilities.
Physical Requirements:
- Work performed in a medical setting due to the need to care for residents in person.
- Must be able to sit, stand, bend, reach, talk, hear, stoop, kneel, crouch, stretch, and twist.
- Ability to assist patients with mobility and transfers during therapy sessions and transportation services.
- Fine motor skills and dexterity for assisting therapists with treatment procedures and operating transportation vehicles.
- Prolonged periods of standing, walking, bending, and reaching.
- Stamina to stand and walk for extended periods while working with patients and transporting them.
- Visual and auditory acuity to monitor patients during therapy and transportation and communicate effectively with staff members and patients.
