Responsibilities:Essential Functions:
• Supervises team members who coach patients through their healthcare journey by providing education about their condition, healthcare system delivery, and clinical programs to decrease the risk of unnecessary emergency room visits or hospital admissions, encouraging use of the right resources at the right time.
• Partners with Care Coordination leaders and Medical Directors to coach team members in assessing unique patient needs and designing individual plans of care, including chronic disease/complex condition management strategies, targeted health literacy and
education materials, social determinants of health resources, and caregiver support plans.
• Supports problem solving within the team through communication, collaboration, data collection, and critical thinking to evaluate potential solutions to further patient’s progress towards clinical care plan goals.
• Helps determine appropriate work assignments to maintain balanced patient caseloads across the team and supports team availability for new assignment intake.
• Ensures clinical team members practice at the top of their scope and upholds the principles of effective labor force utilization.
• Monitors team productivity and established service level objectives. Works with management teams to make operational adjustments as needed to ensure care coordination and transition support for covered populations is efficient, timely, and effective.
• Assists in developing, implementing, and enhancing existing and new care management programs in concert with internal and external stakeholders to improve care outcomes and cost efficiencies.
• Provides concise and relevant information, data findings, and recommendations to leadership to support the development and
execution of value-based strategies and care coordination initiatives that achieve organizational and department goals.
• Ensures compliance with certifications, licensure, and required education for clinical team members.
• Utilizes a data-driven approach with the assigned care team to identify patients most likely to benefit from care coordination services,
including Accountable Care Organization (ACO) alignment, high-risk/high-cost health conditions, 30-day readmission risk, clinical utilization management services, and preventative services.
• Develops collaborative relationships with payors, vendors, partners, stakeholders, and community resource groups.
• Ensures all patient care activities support established clinical standards of care, comply with various regulatory agencies, and consider age-specific, developmental, and cultural needs of the patient population served.
• Collaborates with other department leaders to integrate care coordination functions to achieve team, department, and organizational goals.
• Maintains a high level of proficiency with organizational informational systems, including ELLiE Healthy Planet modules.
• Conducts concurrent and retrospective medical record reviews to validate the effectiveness of the care coordination team’s documentation in the medical record, ensure timely patient communication, and maintain compliance with department policies and system protocols.
• Educates team to encourage proper utilization of clinical resources, effective post-hospitalization patient coaching, making recommendations for patient interventions and payor resources, and guiding team members through complex cases.
• Works with Orlando Health, Inc., Orlando Health Physician Partners (OHPP), and Orlando Health Collaborative Care (OHCC) executives and physician leaders to develop and support adopted strategies for clinical integration and network development.
• Works with leadership to provide support for data review, market analysis, clinical program growth, and provider relationship activities.
• Conducts regular supervision of team members through clinical observation and auditing of clinical documentation.
• Assists Orlando Health Network Medical Directors in research, program development, and ongoing clinical programmatic oversight.
• Directly oversees the value-based care coordination team.
• Manages, mentors, and supportsthe development of clinical team members joining the value-based care team to accelerate learning curves, ensure departmental growth, and aid in succession planning.
• Helps team members meet performance expectations for their roles; provides guidance and recommendations to leverage system and community services, products, and patient resources.
• Supports system-wide clinical transformation activities with multidisciplinary teams to improve quality outcomes and reduce expenditure on value-based contracts.
• Assists in identifying ways to innovate and automate care coordination workflows when available and works with the appropriate technical resources and vendors to do so.
• Interprets clinical analytics reports and works with the analytics team to compile and analyze relevant information to support informed strategic decision-making for clinical teams and initiatives.
• Identifies, evaluates, and understands national and local trends in healthcare, competitor activities, consumer preferences, economic
conditions, reimbursement trends, and regulatory requirements.
• Manages various human resource functions including hiring, work assignments, coaching plans, and performance counseling of team
members.
• Possesses a detailed understanding of health care industry trends including, but not limited to, payment models, care delivery settings, value-based care, population health, and clinical care management.
• Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
• Maintains compliance with all Orlando Health policies and procedures.
Other Related Functions
• Works comfortably in teams as both a participant and facilitator, including temporary teams for project-based initiatives.
• Communicates effectively across all forms of media, uses active listening and communications skills, and leverages presentation skills to engage, inspire, and establish credibility with payer, provider, and employer partners
Education/Training:
• An associate’s degree or higher is required.
• Completion of an accredited Medical Assistant training program is preferred.
Licensure/Certification:
Preferred: Registered or Certified Medical Assistant.
Experience:
Five (5) years of patient care experience in a relevant clinical role is required. Three (3) years in a supervisory role may substitute for the required experience.
