JOB SUMMARY

At Family Tree Private Care the primary purpose of the Director of Nursing is to plan, organize, develop, and direct the overall operation of our Licensed Home Health services. Success in this position is measured by compliance with current federal, state, and local standards, guidelines, and regulations that govern licensed home health. Additionally, success is measured through patient quality outcomes, staff retention, and staff education/performance.

RESPONSIBILITIES

  • Directs and coordinates the regional clinical department personnel, including Clinical Care Coordinators, Care Advisors, Clinical Liaison, and the Assistant Director of Nursing.
  • Assumes the responsibility for continuity, quality, and safety of services delivered in compliance with company guidelines and state regulations.
  • Partners with regional operations and sales teams to meet monthly, quarterly, and annual goals related to private nursing care and personal assistance care.
  • Ensures that private nursing care is coordinated and integrated consistently across the region.
  • Collaborates on the development and implementation of policies and procedures which guide and support the provision of care in accordance with agency policy, procedure, and state regulations.
  • Partners with the Director of Administration to ensure the region maintains compliance with all LHH/PAS regulations and required documentation. Monitors the quality improvement program (QAPI) and assures appropriate corrective measures are performed in accordance with LHH/PAS regulations.
  • Ensures that a private nursing client’s Plan of Care is executed as written and updated as required.
  • Ensures reassessments of client’s care needs are performed when a patient has a health status change, upon the physician’s request, and after a hospital discharge.
  • Conducts field visits to support Clinical Care Coordinators, Care Providers, and patients.
  • Presents a professional appearance and demeanor when visiting new and existing clients to ensure high rate of conversion from prospect to client.
  • Trains and supports Clinical Care Coordinators and Care Advisors to ensure excellent client care and client retention.
  • Addresses client and care provider concerns in a timely manner.

Requirements

REQUIREMENTS

  • RN required. BSN preferred.
  • Strong candidates have minimum of 5 years of experience working in the home care industry (or related industry – hospice, home health, hospital, etc.)
  • Three years minimum of management experience preferred.

SKILLS AND EXPERIENCE

  • Hungry, Humble, Smart.
  • Eager to deliver exceptional care to clients and willing to go above and beyond to satisfy clients and care providers.
  • Fluent in basic Microsoft Office 365 functions (Outlook Calendar, Word, Excel, PowerPoint)
  • Bilingual in Spanish (preferred).
  • Strong organizational and communication skills.
  • Positive team player who can collaborate and partner with all staff and hold team accountable.
  • Ability to balance and prioritize needs of multiple individuals.
  • Available to assist and resolve issues after hours as required.
  • Passion for taking care of older adults and persons with disabilities.
