Job Purpose / Function
The Hepatitis C Case Manager provides direct outreach, testing, and care coordination for individuals at risk for or living with Hepatitis C and related conditions. This role is central to Liberation’s SAMHSA-funded Hepatitis C Elimination initiative and works primarily with people who use drugs and those experiencing homelessness.
The Case Manager identifies, tests, and supports participants through the full Hep C treatment journey — from screening to cure — while connecting them to wraparound supports including substance use treatment, HIV prevention and care, and wound or mental health services.
Major Duties and Responsibilities
- Conduct targeted outreach in community settings, encampments, and shelters, and on mobile health vans to identify individuals at risk for HCV and HIV.
- Administer HCV RNA point-of-care testing (Cepheid) and provide pre- and post-test counseling.
- Enroll participants in EPIC and coordinate confirmatory testing and telehealth treatment initiation with Yale providers.
- Support participants through medication delivery and adherence, including assistance with medication storage for those who are unhoused.
- Provide risk-reduction counseling, overdose prevention education, and supplies.
- Develop individualized case management plans and coordinate linkage to HIV care, PrEP, SUD treatment, wound care, and behavioral health services.
- Track treatment adherence and coordinate confirmatory testing to ensure cure.
- Maintain accurate and timely documentation using EPIC, Cordata, and REDCap.
- Participate in team meetings, case reviews, and data reporting for SAMHSA and Yale partners.

- Bachelor’s degree in social work, public health, or a related field preferred; equivalent combination of education and experience accepted.
- Minimum two years’ experience in outreach, case management, or clinical settings serving individuals who use drugs or are experiencing homelessness.
- Trained in HIV/HCV testing and counseling; phlebotomy or RNA point-of-care testing experience preferred.
- Proven ability to build rapport and maintain trust with participants who may be ambivalent about care or have previously disengaged from treatment.
- Strong follow-through, empathy, and problem-solving skills.
- Experience using EPIC, Cordata, or other EHR systems; proficiency with Microsoft Office Suite.
- Valid driver’s license and reliable vehicle required.
