Job Purpose / Function

The Hepatitis C Case Manager provides direct outreach, testing, and care coordination for individuals at risk for or living with Hepatitis C and related conditions. This role is central to Liberation’s SAMHSA-funded Hepatitis C Elimination initiative and works primarily with people who use drugs and those experiencing homelessness.

The Case Manager identifies, tests, and supports participants through the full Hep C treatment journey — from screening to cure — while connecting them to wraparound supports including substance use treatment, HIV prevention and care, and wound or mental health services.

  • Support participants through medication delivery and adherence, including assistance with medication storage for those who are unhoused.
  • Provide risk-reduction counseling, overdose prevention education, and supplies.
  • Develop individualized case management plans and coordinate linkage to HIV care, PrEP, SUD treatment, wound care, and behavioral health services.
  • Track treatment adherence and coordinate confirmatory testing to ensure cure.
  • Maintain accurate and timely documentation using EPIC, Cordata, and REDCap.
  • Participate in team meetings, case reviews, and data reporting for SAMHSA and Yale partners.
