Job Purpose / Function

The Hepatitis C Outreach Program Manager leads the implementation of Liberation’s Hepatitis C Elimination Program, funded through SAMHSA. The position ensures high-quality outreach, testing, and treatment coordination for people at high risk for Hepatitis C and HIV, particularly individuals who use drugs and/or experience homelessness.

The Manager oversees field operations and data systems while providing direct support to the outreach team. The role blends supervision, field presence, and systems coordination — ensuring people are located, tested, linked to telehealth treatment through Yale, supported through medication adherence, and confirmed cured.

Major Duties and Responsibilities

  • Supervise the Hepatitis C Case Manager and Prevention Outreach Specialists, setting priorities, providing regular supervision, and evaluating performance.
  • Conduct and oversee outreach in encampments, shelters, community sites, and on mobile health vans to identify and engage people at risk for Hepatitis C, HIV, and other infectious diseases.
  • Perform or oversee HCV RNA point-of-care testing using Cepheid machines, as well as HIV and STI testing.
  • Provide counseling on test results and education around risk reduction and overdose prevention.
  • Coordinate enrollment in EPIC, confirmatory testing through Yale, and same-day telehealth visits with Yale providers for HCV treatment initiation.
  • Facilitate medication delivery and storage solutions for participants experiencing homelessness, ensuring continuity of treatment.
  • Oversee adherence strategies, including directly observed therapy (DOT) and incentive-based engagement.
  • Monitor treatment completion and coordinate confirmatory post-treatment testing.
  • Maintain accurate documentation and reporting using EPIC, Cordata, and Redcap.
  • Build and sustain partnerships with Yale, local pharmacies, housing programs, and community providers to ensure wrap-around care (SUD, PrEP, HIV, wound care, behavioral health).
  • Ensure compliance with program, funder, and agency protocols for infection control, confidentiality, and data integrity.

Skills / Experience Required

  • Bachelor’s degree in social work, public health, nursing, or a related field; master’s degree preferred.
  • Minimum of three years’ experience in outreach, case management, or program coordination related to infectious disease, harm reduction, or SUD.
  • Supervisory experience required.
  • Trained in HIV/HCV testing and counseling; experience with Cepheid or other RNA testing platforms preferred.
  • Demonstrated ability to engage individuals with complex health and social needs and to build trust with those who have struggled to complete HCV treatment in the past.
  • Exceptional interpersonal and communication skills, with a deep commitment to dignity, respect, and non-judgmental support.
  • Proficient in electronic data systems (EPIC, Cordata, REDCap) and Microsoft Office Suite.
  • Valid driver’s license and reliable vehicle required.
