- Work cohesively with the Clinical Outreach Team to coordinate transitioning levels of care.
- Develops, implements, manages, and continually evaluates individualized client care plans and discharge plans.
- Maintain regular communication with families, referral sources, probation/parole officers, and other stakeholders, as appropriate and with proper consent.
- Assess each client’s social, employment, and medical needs, and facilitate necessary appointments and paperwork whenever possible.
- Assist clients in accessing community resources such as housing, employment, education, transportation, and financial assistance.
- Advocate for client’s needs within the healthcare, legal, housing and employment systems.
- Completes all documentation required within the outlined timeframes, prior to client’s discharge from the facility.
- Regularly meets with clients to assess progress, at a minimum of once weekly, and adjust care plans, as needed.
- Facilitate Case Management groups, and additional groups as needed. • Provide crisis management and intervention as needed.
- Maintain accurate, confidential, and up-to-date records, and completes charting in the Electronic Medical Record (EMR).
- Monitor compliance with court-ordered, probation, or other external requirements when applicable.
- Provide psychoeducation to clients on recovery-related topics (e.g., relapse prevention, life skills, coping strategies).
- Support client engagement in 12-step, SMART Recovery, or other community-based recovery support groups.
- Coordinate with medical staff regarding medication-assisted treatment (MAT) follow-up and adherence.
- Participate in ongoing training and professional development to remain current with best practices in SUD treatment and case management.
- Provide mentorship and support to Behavioral Health Technicians during cross-trained shifts, promoting consistent client care standards.
- High school diploma or equivalent required
- Minimum of one year of case management or equivalent role experience in a substance use disorder treatment program or other mental health setting required
- Registered through California Consortium of Addiction Programs and Professionals (CCAPP), Addiction Counselor Certification Board of California (ACCBC), California Association for Alcohol/Drug Educators (CAADE), or California Association of DUI Treatment Programs (CADTP) preferred
- Registered Drug and Alcohol Technician (RADT), Certified Alcohol & Drug Counselor Level I (CADC-I) or similar certification preferred
- Certification in approved de-escalation and restraint techniques (e.g., CPI, Mandt, or equivalent), or ability to obtain within 90 days of hire
- Valid CA Driver’s License with ability to pass a Motor Vehicle Record (MVR) check
- Current Adult CPR certification required
- Current Adult First Aid certification required
