Key Responsibilities
- Perform in-home assessments to determine functional impairments and therapy needs
- Develop and implement individualized plans of care that support safety, self-care, and mobility
- Provide hands-on interventions focused on ADLs, home safety, and patient-specific goals
- Educate patients and caregivers in the use of adaptive equipment and techniques
- Collaborate with physical therapists, nurses, aides, and physicians for coordinated care
- Document progress and modify care plans in response to clinical changes
- Support safe transitions and long-term functional wellness
Requirements
What We’re Looking For
- Licensed Occupational Therapist (OTR/L) in the state of New Mexico
- Minimum of 1 year of clinical experience (home health experience preferred)
- Strong problem-solving, communication, and time management skills
- Comfort working independently in patients’ homes
- Experience with EMR systems and mobile charting tools
- Passion for enabling independence and dignity through skilled therapy
