Individual Support Services (ISS) Case Manager
Waystone is currently looking for an Individual Support Services (ISS) Case Manager. Your focus will be to support the independence of individuals with Intellectual and Developmental Disabilities (I/DD). Working collaboratively with families and informal/formal supports you’ll help develop essential life skills and community inclusion.
Location: The position is based out of our Day Program office located in Haverhill, MA. This position involves at least 50% of regular travel throughout Northeast Massachusetts and southern New Hampshire.
Schedule: Core business hours of Monday - Friday 9am-5pm. Flexibility is required evenings and weekends to meet the needs of the individuals we serve. These openings are benefit eligible as of date of hire.

Compensation: The hourly rate is $23.00. You will be a union member to SEIU Local 509.

Job Responsibilities:
- Assist with the development and implementation of Individual Support Plans (ISP) and support individuals in working toward their goals as outlined in the plan.
- Coordinate medical care and appointments, assist with obtaining medications, and provide support around employment, legal matters, and financial concerns.
- Teach essential daily living skills such as household upkeep, personal hygiene, healthy routines (including diet, exercise, and stress management), budgeting, and following medical guidance.
- Conduct ongoing behavioral assessments to evaluate each individual’s ability to manage life challenges and determine appropriate supports.
- Maintain accurate documentation, including weekly and quarterly progress notes, in compliance with DDS regulations.
Additional Responsibilities:
- Conduct routine apartment visits to monitor cleanliness and safety, and provide guidance as needed.
- Help individuals access essential services, including Personal Care Attendant (PCA) support, Visiting Nurse Association (VNA) services, therapy, transportation, medical care, recreation, housing, adult education, and other community-based resources.
- Encourage and support individuals in developing self-advocacy skills to promote independence and personal empowerment.
- Work with parents to strengthen parenting skills and promote positive family dynamics.
Qualifications:
- High school diploma with a minimum of 1 year of experience supporting individuals with developmental disabilities.
- Candidates must be able to read, write, and comprehend English to accurately follow care plans, document services provided, and communicate effectively with individuals, families, and healthcare professionals.
- Strong computer literacy, including proficiency in Microsoft Word and Excel
- CPR/First Aid; training is offered by Waystone to acquire certification within 3 months of employment.
- Valid Driver’s License in good standing with a minimum of one year of driving experience.
