Responsibilities

• Help clients with personal care, hygiene, meal preparation, and medication reminders.
• Support individuals in community integration and participation in social activities.
• Implement individualized service plans and accurately document progress.
• Maintain a safe and clean living environment for clients.
• Communicate effectively with clients, families, and the multidisciplinary team.
• Attend training sessions and comply with organizational policies and regulations.

Requirements

• High school diploma or equivalent required.
• Experience in direct care or related field preferred.
• Strong interpersonal and communication skills.
• Valid driver's license and reliable transportation.
• Ability to work flexible hours, including nights, weekends, and holidays.
• CPR and First Aid certification preferred or must be obtained upon hire.
